- Uses sound clinical judgment, critical thinking and nursing process to provide direct and indirect patient care.
- Maintain and protect the confidentiality of protected health information by preventing
- unauthorized release and/or disclosure, both verbal and written.
- Assesses, monitors, plans and takes appropriate action to prevent potential physiological and/or psychosocial complications while promoting an optimal level of health.
- Is accountable for integration of patient care of all clinical providers within their scope of practice, licensure, relevant education and experience.
- Collects and analyzes patient data, make recommendations for nursing care plan
- Maintains positive interaction with patient, significant others and members of the health care team to effectively provide quality care.
- Triages phone calls and web messages and schedules sick visits, medication check appointments as appropriate.
- Answers calls and messages for advice as appropriate within the scope of nursing practice, consulting physician as needed.
- Processes requests for medication refills according to protocol.
- Schedules Diagnostic Imaging or lab studies based on physician orders or standing orders protocols.
- Coordination of care for patients utilizing other providers and community agencies and services
- Tracks referrals, monitors outstanding labs and diagnostics and follows up with physician and patient as appropriate
- Assist patients as needed, with physician directed discharge instructions, visit summary plan of care.
- Identification of complex patients and subsequent referral to case manager as appropriate
- Oversees immunizations, ordering, storage, monitoring temperatures, inputting and tracking lot numbers and supply.
- Is familiar with immunization billing codes and use of both State and Private supplies
- Assists with staff education on various clinical topics and redesign of clinical workflow as needed.
- Provides leadership by delegating, supervising and participating in safe nursing care activities provided by ancillary personnel.
- Evaluates the effectiveness of patient care and identifies opportunities for improvement.
- Collaborates with the healthcare team, patient, family members and community services regarding the patient care outcome.
- Access and provide appropriate teaching based on patient and family needs.
- Participates in site-based quality improvement and research activities (if applicable).
- Utilizes all available resources to provide quality, cost-effective patient care.
- Meets and maintains clinical competencies measured by pre-determined credentialing standards, mandatory/safety education and completion of Basic Life Support and ACLS/PALS as appropriate.
- Identifies growth needs and participates in educational opportunities and professional organizations.
- Maintains medical supply inventory.
- May answer phones, meet/greet patients, collect co-pays and schedule appointments.
- Serves as a role model and trains less experienced personnel.
- Demonstrates flexibility to perform duties wherever volume deems it necessary within the office.
- Follow OSHA standards regarding blood borne pathogens and practice Universal Precautions. Adhere to safety guidelines at all times.
- Stand by and/or assist with patient examination and/or procedures as needed.
- Assist patients to sign up for the patient portal, reset passwords, encourage patients to participate in the RI Health Information Exchange
- Attend meetings.
- Baccalaureate in Nursing preferred.
- RN Licensure in the state of Rhode Island required.
- AHA Basic Life (CPR)
- Experience in a physician practice group setting preferred.
- Previous dermatology experience, specifically in cosmetics, preferred.
- Demonstrated computer proficiency
- Demonstrated knowledge and skills necessary to provide care to patients throughout the lifespan, with consideration of the aging process, human development stages and cultural patterns in each step of the care process.
- Receptivity to learning opportunities within the workplace, accept constructive feedback from other team members in an effort to collaborate and improve the efficiency and effectiveness of patient care.
