Pediatric Hematology Nurse Navigator - Pediatric Hematology/Oncology

Summary:
The purpose of the pediatric hematology nurse navigator is to: 

  1. Help pediatric hematology patients navigate the healthcare system with a focus on inpatient to outpatient transitions
  2. Personalize the care our patients receive
  3. Provide an educational resource for patients and families
  4. Provide consistent contact and communication for pediatric hematology patients

The nurse navigator facilitates care of the patient and family across the many disciplines and professionals required for their care. The nurse navigator serves as the liaison for the patient and family addressing their concerns and questions and to coordinate patient care.


Responsibilities:

1. Communication: 

  • Nurse Navigators will ensure that there is continuity of communication among the patient's medical team (both internal and external to UNC Health). 

2. Nurse Navigator Continuing Education: 

  • Serves as an educator on the healthcare team for hematology-specific education and disease management. 

3. Patient Care Duties: 

  • Interviews patients regarding their current health practices, identifies health problems, determines the degree of compliance and investigates barriers with adherence of the prescribed treatment plan, and documents this information in the patient's chart. 
  • Analyzes data from patient interviews, chart reviews, and physical assessments to determine patient's nursing care needs including direct care and referral to other departments. Coordinates scheduling of appointments, labs, infusions, imaging, and consults. Interacts with referring physician's offices for new and ongoing patient care issues. 
  • Obtains prior authorization and supports approval of new medications. 
  • Assists with referrals, home health, and school forms. 

4. Programmatic Leadership: 

  • Functions as part of a comprehensive hematology team. 
  • Provides cross coverage for other team members and assists with process development, implementation of new programs, and other initiatives.

● Bachelor's degree in Nursing (BSN) from a state-accredited school of professional nursing.

● Two (2) years of nursing experience and one (1) year in the area of specialization.

Qualifying candidates will receive a $15,000 Commitment Bonus! Unit Summary: NP 5200 is a 37-bed inpatient adult medical/stepdown oncology unit. Our main patient populations involve symptom management for oncology patients, administration of chemotherapeutic agents, and end-of-life care. Nursing care is provided in collaboration with other departments and disciplines including hospital medicine, surgical services, pharmacy, case management, social work, nurse navigators, dieticians, and chaplain services. The unit is also part of Duke Cancer Institute of Wake County, which provides comprehensive cancer treatment for patients in Wake County receiving care across the continuum of inpatient and ambulatory settings. Work Hours & Shifts: 12-hour shifts (7A to 7P) Weekend expectation: up to 96 hours per 8-week schedule Major holidays are assigned upon hire on a 3 year rotating schedule Job Summary: Plan and provide professional nursing care for patients in accordance with physician orders and established policies and procedures. RNs with no previous oncology/chemo experience will be skilled to administer chemotherapy/immunotherapy within 6-8 months of hire. We offer didactic education and hands-on precepted orientation for chemotherapy/immunotherapy administration based on ONS/APHON guidelines, DUHS policies and procedures, and appropriate evidence in practice standards. RNs will complete the ONS/ONCC Chemotherapy Immunotherapy Certificate Course within 24 months of initial chemotherapy/immunotherapy competency with DUHS. Job Duties: Plan and provide professional nursing care for patients in accordance with the scope of the RN as defined by the North Carolina Board of Nursing Perform other related duties incidental to the work described herein. All registered nurses without a Bachelor's degree in Nursing (or higher) will be required to enroll in an appropriate BSN program within two years of their start date and to complete the program within five years of their start date Nurses hired at Duke Health with a valid compact license are required to obtain their North Carolina nursing license within 90 days of their start date.
