The Solid Organ Transplant Coordinator is a registered nurse who plans, coordinates and implements the care of the solid organ transplant patient and living donor as applicable in both the in-patient and out-patient settings in accordance with the nursing process. The scope of the role includes acting as liaison with the organ procurement organization and the OPTN, assesses human, financial and material resources of patients and families; coordinates information flow among medical, ancillary, nursing, financial and insurance company staff; represents the transplant program through involvement in hospital and community activities; participates in the development and maintenance of the transplant statistical database. Employee will recognize the population specific needs of the transplant patient and apply the appropriate knowledge, skills and behaviors to meet those needs when performing patient activities.

What you will do in this role: Assesses patient pain regularly to promote effective pain management, including reassessments after appropriate intervention. Performs procedures, monitoring, or other functions as ordered by the medical provider(s). Documents the administration of care in the patient medical record in a timely and thorough manner. Performs exceptional care by responding promptly to patient requests. Strives to anticipate patient needs and resolve them proactively. Teaches patients/families/caregivers about patient medical condition, patient status, treatment plan, medications and possible side effects, and follow-up measures. Translates complex medical terminology to ensure complete understanding. Teaches patients/families/caregivers about any non-medicinal follow-up measures, such as healthy diet and exercise, disease prevention, and/or other lifestyle changes.
