Responsibilities for Licensed Practical Nurse (LPN):

Direct the day-to-day functions of the nursing assistants. Meet with your assigned nursing staff, as well as support personnel, in planning the shift’s services, programs and activities.
Make written & oral reports/recommendations concerning the activities of the shift as required. Admit, transfer and discharge residents as required. Provide leadership to nursing personnel assigned to your unit/shift.
Prepare & administer medications as ordered by the physician.
Develop & maintain a good working rapport with inter-departmental personnel, as well as other departments within the facility to ensure that nursing services and activities can be adequately maintained to meet the needs of the residents.
Monitor your assigned personnel to ensure that they are following established safety regulations in the use of equipment and supplies.
Maintain confidentiality of all resident care information including protected health information.

Qualifications for Licensed Practical Nurse (LPN):

Current Illinois Nursing License (Required)
Current CPR certification (Preferred)

Rates Range: $31.00 - $36.00 (Hourly) Dependent on Experience.

Basic Function The primary purpose of Wound Care Nurse is to provide for the day-to-day care needs of the residents in a Skilled Nursing Facility Environment. Essential Duties 1. Provide hands on care to the resident based on the resident Plan of Care. 2. Must be able to identify changes in the resident condition and evaluate the resident care needs. 3. Strong focus assessment skills as well as documentation skills. 4. Must be able to evaluate and participate in the development of the Resident Plan of Care. 5. Monitor the resident overall condition and provide care as required while maintaining compliance with the facility policy and procedures. 6. Provide primary skin care to residents under the medical direction and supervision of the residents’ attending physicians, or the Director of Nursing Services, with an emphasis on treatment and therapy of skin disorders. 7.
