Responsibilities

  • Administer medications and treatments per care plans and physician orders

  • Monitor, document, and report changes in resident condition

  • Participate in care planning and ensure resident satisfaction

  • Supervise CNAs and ensure quality care standards

  • Follow all local, state, and federal regulations

Qualifications

  • Active LPN license in the state of Wisconsin

  • Ability to work independently and as part of a team

  • Basic computer skills (EMR experience preferred)

  • Long-term care experience preferred, but not required

  • Compassionate, patient-centered approach
