Assist with daily living activities, including personal care, and mobility assistance.

Obtains urine/stool/sputum specimens. Transfers, moves and repositions patients. Makes occupied and unoccupied beds. Maintains current knowledge and certification requirements. Receive written/verbal report at the beginning of each shift regarding the care that is to be delivered to the assigned group of patients. Give report when leaving the unit, and at the completion of the shift regarding the care that was delivered; any abnormalities that occur during the shift are to be reported to the RN as well as to the oncoming CNA/NA Assist with data collection i.e.. Vital signs, weight, blood sugar, functional status and communicates changes to the RN in a timely fashion. Participate in respectful communication with patients, families and coworkers. Observe skin changes and reports to RN Promote infection control by using appropriate hand washing techniques. Follow standard precautions and any other isolation precautions as dictated by patient condition. Maintain a safe environment and protects patients from injury by observing the patient who may be at risk for falls, confusion, disorientation or suicidal behaviors. Perform patient care rounds Communicate pertinent observations to appropriate members of the health care team in a timely fashion. Work in collaboration with all members of the health care team.
