Registered Nurse (RN) Supervisor/Admissions Nurse

Position Overview:
This role is based at a healthcare facility in the Bronx, supporting admissions, care planning, and overall nursing department operations while ensuring high-quality resident care.

Admissions experience is a must!

Duties & Responsibilities:

  • Conduct admission assessments for new residents and hospital returns; initiate care plans.

  • Review PRI documentation and coordinate all aspects of the admission process with the interdisciplinary team.

  • Assist in the development, implementation, and oversight of nursing policies, procedures, and department operations in collaboration with the Director of Nursing (DON).

  • Schedule and assign nursing staff to effectively meet resident care needs.

  • Foster a compassionate environment by supporting residents and their families.

  • Provide information and respond to questions from residents, families, and staff.

  • Review and revise resident care plans based on changes in condition.

  • Participate in discharge planning as needed.

  • Complete MDS, charting, and all required documentation in accordance with policies and procedures.

  • Audit resident and departmental records to ensure compliance with facility standards and regulatory requirements.

  • Maintain a safe, clean, and compliant working environment by adhering to established guidelines.

  • Ensure confidentiality and proper handling of resident information in accordance with HIPAA and corporate policies.

  • Maintain nursing supply inventory and related operational needs.

  • Supervise or direct resident care as assigned.

Requirements/Qualifications:

  • Active, unrestricted New York State Registered Nurse (RN) license.

  • Long-term care (LTC) experience required; admissions experience needed.

  • Strong clinical assessment and critical thinking skills.

  • Ability to make sound, independent decisions.

  • Demonstrated leadership and supervisory abilities.

  • Basic computer proficiency; familiarity with Microsoft Office (Excel, Word, Outlook) preferred.

  • Excellent communication and interpersonal skills.

  • Strong team player with the ability to collaborate across departments.
