Description:

Per Diem RN HOSPICE for Nursing Homes in the Saratoga/Washington County Region.

The Community Hospice has a wonderful opportunity for a Per diem RN to work as part of our Hospice Care Team caring for patients and families in local Nursing Homes.

Needs are on Weekdays (8a-4:30p) as needed. No Weekends available

What you will do:

  • You will case manage patients in the Nursing home setting and respond to symptom management needs of our Hospice patients and coordinate with our team to provide added support to residents and families.
  • Work with interdisciplinary team.

What you will need: 

  • Valid NYS RN license required.
  • Minimum 1 year RN experience required. 1-2 years long term care RN experience preferred.
  • Previous Hospice experience preferred.
  • Ability to learn/use an electronic medical record
  • Clean Driver's License and reliable vehicle
  • Strong interpersonal skills to establish and maintain relationships with patients, families, physicians and coworkers

Pay Range for non-per diem full-time or part-time Hospice RN employment: $38.00-$50.18 Pay is based on experience, skills, and education. The pay range may also vary within the stated range based on location.
