Position Summary

The Registered Nurse (RN) Care Manager is responsible for coordinating, implementing, and overseeing personalized care plans for clients receiving in-home support services.

This role ensures that clients receive high-quality, person-centered care while maintaining their overall health, safety, and well-being. The Care Manager serves as the primary point of contact between clients, families, caregivers, and healthcare professionals, ensuring services are delivered effectively and in compliance with all regulatory and company standards.

This position requires strong clinical judgment, organization, communication, and leadership skills, along with the ability to manage multiple priorities in a fast-paced environment.

Minimum Qualifications

  • Active Registered Nurse (RN) license in the State of California (Required)
  • Minimum 2 years of experience in home care, home health, hospice, or related field
  • Valid driver’s license, reliable transportation, and current auto insurance
  • Excellent written and verbal communication skills
  • Strong attention to detail, follow-up, and organizational skills
  • Ability to problem-solve, take initiative, and exercise sound clinical judgment
  • Experience supervising caregivers and advocating for clients and families
  • Professional, positive, and client-centered demeanor
  • Ability to pass a criminal background check
  • Bilingual (English/Spanish) is a plus
Essential Duties & Responsibilities

Client Care Management

  • Conduct initial and ongoing client assessments
  • Develop, implement, and update individualized care plans
  • Monitor service delivery to ensure quality and consistency
  • Maintain accurate and timely documentation
Caregiver Oversight & Support

  • Supervise caregivers to ensure:
    • Compliance with care plans
    • Proper infection control practices
    • Timely reporting of changes in client condition
    • Adherence to emergency protocols
  • Provide coaching, guidance, and support to caregivers
  • Ensure caregivers have the appropriate skills for each case
Client & Family Coordination

  • Serve as the primary point of contact for clients and families
  • Facilitate caregiver introductions for first shifts
  • Review care plans with clients and caregivers and set expectations
  • Ensure care plan documentation is accessible in the client’s home
Operational Responsibilities

  • Assist with caregiver scheduling coordination
  • Collect fees for initial services at the time of assessment or prior to the first day of service
  • Ensure compliance with company policies and regulatory requirements
Compensation & Bonus Structure (Premium Tier)

  • Pay Range: $50 – $70 per hour (based on experience, clinical background, and leadership capability)
  • Monthly Performance Bonus: $750 – $3,000+
    • Based on client outcomes, care plan compliance, documentation quality, and family satisfaction
  • Case Load Bonus: Tiered incentives for managing higher-acuity or higher-volume caseloads
  • Referral Bonus: $300 – $750 for each new client referral that starts services
  • Retention Bonus: $150 per client retained beyond 90 days
  • Leadership Incentive (Senior/Lead RNs): Additional $1,000 – $3,000/month based on team oversight and performance metrics
Total compensation is performance-driven and scales with caseload, leadership contribution, and client outcomes.

PURPOSE AND SCOPE: The professional registered nurse Outpatient RN CAP 1 is an entry level designation into the Clinical Advancement Program (CAP). EXPERIENCE AND REQUIRED SKILLS: · Entry level for RNs with less than 2 years of Nephrology Nursing experience as a Registered Nurse. · Chronic/acute hemodialysis experience (preferred). The rate of pay for this position will depend on the successful candidate’s work location and qualifications, including relevant education, work experience, skills, and competencies.
