Home Health Registered Nurse (RN)

The RN is responsible for assessing patient health problems and needs, developing and implementing nursing care plans, and maintaining medical records. May advise patients on health maintenance and disease prevention or provide case management. Licensing or registration required. 

ESSENTIAL DUTIES AND RESPONSIBILITIES 

The essential functions include, but are not limited to, the following: 

  • Provide skilled nursing treatments as ordered by the client’s doctor
  • Available on Monday, Wednesday, and Friday 
  • Complete evaluation tasks, including vital signs and medication reviews 
  • Coordinate with physicians, therapists, and other individuals in the client’s care plan 
  • Assess and chart observations of the client’s condition at each visit 
  • Maintain complete and accurate documentation of nursing care 
  • Work collaboratively with all members of the healthcare team 
  • Provide education to clients and families on proper home health care procedures and strategies 
  • Record patients' medical information and vital signs 
  • Monitor, record, and report symptoms or changes in patient's conditions 
  • Administer local, inhalation, intravenous, or another anesthetic 
  • Prepare patients for and assist with examinations or treatments 
  • Modify patient treatment plans as indicated by patients' responses and conditions 
  • Order, interpret, and evaluate diagnostic tests to identify and assess the patient's condition 
  • Monitor all aspects of patient care, including diet and physical activity 
  • Engage in research activities and participation in formal educational training activities related to nursing 
  • Perform other duties as assigned 

MINIMUM QUALIFICATIONS (KNOWLEDGE, SKILLS, AND ABILITIES) 

  • Associates degree required; Graduate of an accredited state-approved, Bachelor’s Degree from the school of nursing preferred 
  • Current and valid Connecticut Registered Nurse License 
  • Prior nursing experience, specifically in home health (preferred) 
  • Current CPR certification (required), Driver’s License, Car Insurance, Car Registration 
  • Strong interpersonal skills that build personal rapport and establish trust with the client 
  • Computer literate 
  • Strong organizational skills 
  • Excellent written and communication skills 
  • Ability to meet physical demands of the job (ex: lifting 50 pounds for transfers) 
  • Home health care experience 
  • Bilingual (Spanish) preferred
