Seven Hills Rhode Island is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team! Responsibilities
Healthcare & Emergency Care: Provide direct healthcare and emergency care for people supported by Seven Hills Rhode Island, ensuring their health needs are met at home and in the community.
Health Assessments: Complete initial and annual health assessments for participants, ensuring up-to-date care plans tailored to their specific needs.
Health Education: Educate participants and staff about health maintenance, medication administration, and specialized procedures.
Documentation & Records: Maintain confidential health records in accordance with regulations, ensuring all documentation is timely and accurate in the electronic health record system.
Medication & Treatment: Train non-nursing personnel on proper medication and treatment administration, and oversee their compliance.
Multidisciplinary Team Collaboration: Work with a team of healthcare professionals, families, and agencies (e.g., BHDDH) to ensure continuous care and support.
On-Call Responsibilities: Participate in the Nurse on-call system to provide 24-hour telephonic coverage on a rotating schedule.
Training & Support: Assist in the training and support of Direct Support Professionals (DSPs) and other team members on healthcare needs, procedures, and safety protocols.
Medical Appointments: Provide transportation to medical appointments, if necessary, and assist with scheduling and follow-up care.
Regulatory Compliance: Ensure all activities comply with the Behavioral Healthcare, Developmental Disabilities, and Hospitals (BHDDH) standards and regulations.
Qualifications
Qualifications:
High school diploma or GED required.
Licensed Practical Nurse (LPN) with current license in Rhode Island.
Basic Life Support (BLS) certification required.
1-3 years of experience in nursing, particularly with adults with intellectual or developmental disabilities, preferred.
Valid driver’s license with an acceptable driving record.

Clinical Competence: Ability to provide basic medical care and emergency interventions with professionalism and expertise.
Teamwork: Ability to work well with a multidisciplinary team to meet the healthcare needs of participants.
Compassionate Communication: Excellent interpersonal skills to communicate effectively with participants, families, and team members.
Problem-Solving: Use independent judgment and initiative in assessing health needs and making decisions.
Organizational Skills: Capable of managing multiple tasks and maintaining accurate documentation.
Ethical Practice: Commitment to maintaining confidentiality and following ethical guidelines in healthcare delivery.
