Responsibilities

  • Coordinates and oversees all aspects of patients' home care.
  • Collaborates with patient care teams to develop and implement care plans.
  • Educates families on patient condition, needs and care.
  • Performs head to toe physical assessments.
  • Checks vital signs and manages medications.
  • Gives injections, IV therapy or tube feedings.
  • Administers and maintains catheters.
  • Completes documentation timely and accurately.
  • Supervises LPNs and HHAs.
  • Regularly communicates patient progress to the clinical manager, physician and care team.
  • Other duties as assigned.

Qualifications

  • One year of clinical experience as a registered nurse (RN). If less than 1 year clinical experience as a RN, candidate must be approved by VP clinical.*
  • Current RN license, specific to the state(s) you are assigned to work.
  • Minimum six months home health experience.
  • Current CPR certification.
  • Valid driver's license, reliable transportation and liability insurance.
  • Able to assess patient status and identify requirements relative to age specific needs.
  • Must be familiar with general use and functions of the computer, such as, user names and password concepts; internet; e-mail; navigation of computer desktop, including starting programs, using files and windows, effectively use navigation buttons and tool bars; ability to self-manage online HR services and online training programs.

*Note - If less than 6 months clinical experience as a RN, candidate must participate in RN Intern program.

Our compensation reflects the cost of labor across several U.S. geographic markets and may vary depending on location, job-related knowledge, skills, and experience.

The Registered Nurse assumes responsibility for professional development, continuing education and maintaining licensure. Essential Functions: Utilizes the nursing process: assessment, planning, intervention, implementation, and evaluation and formulating nursing interventions directed towards development of an individualized plan of care and treatment modalities. Identifies risk levels, precautions, and expected outcomes individualized to the client to include discharge planning; prescribes interventions to attain expected outcomes; evaluates the effectiveness of interventions in relation to expected outcomes; conducts ongoing assessment of clients from admission to discharge; initiates medical/psychiatric crisis intervention to stabilize and ensure the well-being of the patient's conditions. Demonstrates knowledge of psychiatric disorders and appropriate nursing interventions; implements nursing interventions identified in the plan of care that is individualized to the patient needs. Applies appropriate interventions to progress the patient towards wellness and addresses maintenance of wellness. Administers medications as prescribed safely; provides education on prescribed medications; recognizes signs and symptoms of adverse drug reactions. Demonstrates professionalism, accountability, and responsibility for decision making, quality nursing care, and knowledge base of psychiatric nursing care. Reviews, revises, and updates medical records within facility guidelines and timeframes. Transcribes and implements physician's orders according to facility policy; documents care, progress, response to intervention in medical record in an accurate, thorough and concise manner. Follows up on client care responsibilities, communicates incomplete assignments, and current client needs to staff to assure continuity of care. As the Weekend Registered Nurse, you will assume full nursing responsibility for the delivery of the Plan of Care for each home health patient, while continuously evaluating personal and professional performance and making necessary changes to increase productivity and quality of care delivered.
