Assesses hospice care patients and families to identify the physical, psychosocial, and environmental needs of patients as evidenced by documentation, clinical records, Interdisciplinary Team reports, after hours reports, and on-site evaluations. Assesses patient needs and obtains data on physical, psychological, social, and spiritual factors that may influence patient/family/caregiver health status. Communicates significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and/or other personnel involved with patient care. Counsels, instructs, and includes the patient, facility, and family in following the Interdisciplinary Plan of Care. Teaches hospice philosophy and pain and symptom management to staff in alternate care settings (nursing homes, hospital, assisted-living facilities, etc.). Provides appropriate support at time of death and perform bereavement assessment. What is Required? Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing Current State License as a Registered Nurse RN 1 year of experience in a clinical care setting Experience in a hospice or home health environment is preferred 50% travel required Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment Valid driver’s license and insurance and reliable transportation to perform job tasks You will report to the Clinical Team Manager. Responsibilities Cedar Creek Hospital of Michigan is a beautiful and state of the art 72-bed facility. Cedar Creek provides evidence-based treatment to adolescents and adults struggling with mental health issues. Cedar Creek Hospital invests in our patients by offering an integrated medicine approach that provides the latest therapies including yoga, mindfulness, aromatherapy, music therapy and nutrition. As part of our success and innovation in the field we are seeking qualified applicants for an Intake Nurse position. Information is normally obtained through initial clinical phone information, followed by clinical evaluation if appropriate for the client and coordinates with the physician regarding patient care needs. The Intake RN will facilitate this process by assisting the client, family and referral sources. They also provide information to clients, families and referral sources regarding treatment programs, insurance and admission procedures.
