Charge Registered Nurse - RN

PURPOSE AND SCOPE:

Functions as the hemodialysis team leader in the provision of chronic hemodialysis care and treatment.  Provides day to day direction and supervision to assigned direct patient care staff.  Communicates with the physician and other members of the healthcare team to interpret, adjust, and complete patient care plans.  Provides safe, effective delivery of patient care in compliance with standards outlined in the facility procedure manual, as well as regulations set forth by the company, state, and federal agencies. Supports FMCNA's mission, vision, core values and customer service philosophy.  Adheres to the FMCNA Compliance Program, including following all regulatory and FMS policy requirements. 

 

PRINCIPAL DUTIES AND RESPONSIBILITIES:

 

General and Staff Related:

  • Provide day to day guidance, support and direction to direct patient care staff, providing informal feedback on an ongoing basis and formal feedback input for the annual performance evaluation.  Participate in the department staffing and the appropriate hiring, firing and disciplinary actions. Recommend disciplinary action to Clinical Manager and initiate as appropriate.

  • Ensure compliance with all company and facility approved procedures and policies as well as regulations set forth by state and federal agencies in clinics with more than 100 patients.

  • Approve or disapprove time or personnel schedule changes in the absence on the Clinical Manager ensuring compliance with applicable regulations, policies and procedures for documenting time of work hours for staff members on assigned shift.

  • Participate in patient care plan meetings. 

  • Maintain knowledge in the current practices related to the principles and techniques of dialysis by participating in all scheduled in-services. 

  • Train and orient staff as necessary.

  • Routinely observe and guide direct patient care staff for appropriate technique and adherence to facility policies and procedures. 

  • Promote and assist with compliance to OSHA programs in order to maintain a safe and clean working environment. 

  • Maintain overall shift operation in a safe, efficient, and effective matter. 

  • With Clinical Manager conduct staff meetings at least monthly or as needed to keep patient care staff informed of changes in patient care needs or operations to improve delivery of care. 

  • Meet routinely with the Clinical Manager to discuss personnel and patient care status, issues, and information.

  • Supervise all documentation of patient information.

  • Coordinate Charge Nurse duties with Staff RNs acting in the relief charge capacity. 

    Patient Care:

  • Assess daily patient care needs and develop and distribute patient care assignments appropriately. 

  • Assume primary responsibility in an emergency situation. 

  • Assess patient needs, respond to dialysis treatments, and communicate concerns to rounding physician.  Implement changes in patient care/treatment as directed. 

  • Monitor and supervise all patient care activity during dialysis and assist as necessary.

  • Collaborate with direct patient care team in making decisions to benefit patient care.

  • Continuously monitor patient's condition with regards to problems and potential complications associated with dialysis. 

  • Administer medications to patients per physician's orders. 

  • Act as the subject matter expert and as a resource for staff members. 

  • Supervise and participate in completion of short- and long-term care plans. 

  • Admit new patients according to facility procedure.

  • Ensure educational needs of patients are met and educate the patient and family about End Stage Renal Disease, dialysis therapy, diet and medication. 

Technical:

  • Supervise the safe and effective use of all equipment involved in direct patient care. 

  • Operate all dialysis related and emergency equipment safely and efficiently when needed.

  • Perform required testing and verification and initial the checklist for start-up and shut-down procedures as outlined in the Technical Services Manual. 

  • Complete Nurse's Technical Training Program/Water Quality Facility Training.

Other:

  • Assist with special projects or other duties as assigned by the Clinical Manager

  • Assist with the interviewing of potential direct patient care staff as requested.

  • Promote efficient use of medical supplies. 

  • Attend and participate in monthly Quality Assurance meetings. 

  • Other duties as assigned.

SUPERVISION:

  • Direct Patient Care Staff, Ward Clerk as assigned.

 

EDUCATION:

  • Graduate of an accredited school of Nursing (R.N.)

  • Current appropriate state licensure.

 

EXPERIENCE AND REQUIRED SKILLS:

  • Minimum of 9 months of nursing experience and an additional 3 months of clinical experience in dialysis

  • RN charge nurses assuming responsibility for nursing and patient servicesin the absence of the Clinical Managermust have one-yearclinical experience and six months dialysis experience.

  • Supervisory or management experience preferred. 

  • Successfully completea training course in the theory and practice of hemodialysis.

  • Good communication skills - verbal and written. 

  • Must meet appropriate state requirements (if any).

The rate of pay for this position will depend on the successful candidate’s work location and qualifications, including relevant education, work experience, skills, and competencies.

Annual Rate: $85,000.00 - $143,000.00

Responsible for the planning, delivery and management of direct patient care utilizing the nursing process and adhering to the standards of nursing practice. I. Major Responsibilities: 1. Utilizes the Nursing Process for the delivery and management of patient’s care from Admission to Discharge, appropriate to the developmental age of the patient population. 2. Performs patient assessment/reassessment according to standard procedure. 3. Develops and coordinates individual plan of care including discharge planning. 4. Directs, supervises, coordinates and evaluate nursing care within assigned clinical setting. 5. Delegates appropriately to others in accordance with job description and competency level. 6. Documents patient care according to standard procedure. 7. Develops, implements/coordinates and documents Education Plan for patient/family/ significant other. 8. Ensures quality and continuity of care through collaboration with other health care providers. 9. Administers medications, performs treatments, therapies and operates equipment according to policy and procedure. Assists physician with treatment and procedures and special tests as required or directed. 10. Participates in and supports Nursing Governance activities. 11. Assumes responsibility for professional growth. Assists in education and development of staff. 12. Position Qualifications: License/Certification/Education: Required: 1. Current Massachusetts Nursing licensure and registration. 2. BLS is a requirement for every RN unless advance cardiac life support (ACLS) certification is current, in addition to any unit specific certifications and competencies. Preferred : 1. Bachelor of Science degree in Nursing highly preferred. Experience/Skills: Required: 1. Thorough knowledge of the discipline of Nursing. Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements. Department-specific competencies and their measurements will be developed and maintained in the individual departments. GENERAL SUMMARY: The Registered Nurse is responsible for the planning, delivery and management of direct patient care using the nursing process and adhering to the standards of nursing practice within a specific unit or clinical area. PRINCIPAL DUTIES AND ESSENTIAL FUNCTIONS: Nursing Process: Utilizes the nursing process for the delivery and management of patient’s care. Maintains assignment of patients and caseload according to unit guidelines and established standards of practice. Documents the patient assessment, plan of care, and response to intervention. Meets all unit documentation standards. Communicates and coordinates the patient’s plan of care to ensure continuity of care. Maintains competency in management of clinical emergencies. Provides complete communications in SBAR format (Situation, background, assessment, recommendations) to relieving staff members and demonstrates advanced planning for the patient. Navigates through the critical elements of the patient medical record software and HealthAlliance-Clinton Hospital Intranet. Clinical Practice: Demonstrates/applies the skill and judgment necessary to implement the medical/nursing plan of care, nursing interventions and procedures necessary for the care of the patient. Bases clinical practice on evidence, unit guidelines, and professional standards to improve the quality of care and to guide clinical judgments. Demonstrates essential knowledge of pharmacology/medications in administering drugs and monitoring the patient’s response. Maintains competency with technology and equipment. Utilizes equipment in a safe manner. Uses appropriate infection control and safety practices in all duties. Acts as a charge nurse as requested by the nursing management team. Collaboration: Delegates responsibilities to others according to Massachusetts general laws and practice guidelines with an understanding of their roles, knowledge and capabilities. Facilitates cooperative and collaborative relationships among the various disciplines and departments to ensure effective quality patient care. Works weekends, holidays and off-shifts as required by unit staffing needs. Ability to read, write and speak clearly in English. EDUCATION: Must be a graduate of a recognized school of Nursing A Bachelor’s degree (BSN) in Nursing is preferred. EXPERIENCE: 2-3 years of Nursing experience preferred.
