Facilitates the physician care plan for a patient's hospital encounter. Using a collaborative interdisciplinary approach, confirms the plan of care and focus on anticipation, integration and coordination of clinical care necessary to meet the needs of patients in order to optimize clinical and resource outcomes. Collaborates with Physician offices, Physician Advisors, Point of Entry Case Managers, Unit-Based Case Managers and Case Management leadership in assessing patient needs and developing the plan of care. Acts as an advocate for the patient, physician and hospital relative to compliance with federal, state and third-party utilization management regulatory requirements.
