How you’ll make an impact in this role

Intake and Admission Coordination:

Manage patient admissions/transfers based on acuity, bed availability, and service needs.

Collaborate with staff to secure the appropriate level of care for optimal outcomes.

Act as a liaison for specialty and consultant physician requests.

Initiate care management, including insurance verification and pre-registration.

Monitor managed care changes and communicate financial/procedural impacts to requesting physicians.

Relay essential medical documentation from referring entities to the nursing unit.

Referral Management and Outreach:

Cultivate relationships with key referral sources and admission decision-makers.

Follow up with referring entities to ensure easy access and support marketing, quality, and evaluation efforts.

Maintain comprehensive documentation of all physician and service referral communications for legal, statistical, medical, and process improvement.

Coordinate training for internal/external departments on the intake review and approval process.

What minimum requirements you’ll need

Licensure / Certification / Registration:

  • BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
  • Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required. Licensure from the Wisconsin Board of Nursing OR current home state license if considered multi-state/Compact State.

Education:

  • Diploma from an accredited school/college of nursing OR required professional licensure at time of hire.

What additional preferences we're seeking

  • Previous experience in long term care/acute care/inpatient rehab setting preferred.
