We are currently seeking Nurse Practitioners provide medical services to patients living in long term care facilities.

NP ROLE AND RESPONSIBILITIES:

Provide primary and preventative care for long term care patients residing in skilled nursing facilities.
Manage Residents with acute change in condition.
Advanced care planning and chronic care management.
Prescribe appropriate medications, including controlled substances in accordance with OH Law.
Order, review and/or follow-up with treatments and diagnostic testing.
Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.
and/or the care plan.
Collaborate with primary care physician.

NP QUALIFICATIONS AND EDUCATION REQUIREMENTS

Current, unrestricted NP license in the state of Ohio.
Board Certification.
DEA licensure.
Clinical background in adult, family or geriatric specialties.
Experience working in a long-term care setting.
Ability to work with an EMR.
Ability to manage patients with advanced illness and multiple chronic conditions.
Ability to work within an interdisciplinary care team to coordinate patient care.
Psych experience a plus

Delivers direct patient care consistent with the advanced practice registered nurse’s education, experience and certification, in collaboration with a physician, which includes diagnosis, management and/or referral of health care issues. Promotes consumer, family, and community health through preventative health care actions and through patient advocacy. The advanced practice registered nurse may also manage complex illnesses and/or deliver specialty care. Upholds the mission, vision, values, and customer service standards of The MetroHealth System. Metro Life Flight: Responsible for advanced emergency and critical care patient management during air and ground transport of the critically ill or injured patient from the scene or outlying sites to participating secondary and tertiary care institutions. Provides intensive care monitoring, critical care intervention and advanced surgical skill procedures to critically ill and injured patients. This includes the management of high-risk obstetrical, medical, surgical, respiratory and cardiovascular patients. The specialized training of the Flight Nurse Practitioner allows for the provision of care to critically ill and injured patients through the use of ventilatory support, continuous cardiac and hemodynamic monitoring, including intra-aortic balloon pump.
