The Certified Nursing Assistant and Home Health Aide plays a critical role as a member of the Interdisciplinary Group Team. You will work under the supervision of a Registered Nurse and perform various services for patients to meet their personal needs and promote comfort.

As a Sweet Dreams CNA You Will: 

(includes but not limited to)

  • Spend quality one on one time with patients and their families to support everyday needs
  • Care for hospice patients by providing paths, back rubs, oral hygiene, shampoos, and changing bed linen as often as needed
  • Provide bedtime care including personal hygiene regimens as required
  • Provide aroma therapy and gentle touch
  • Provide lotion rubs to hands and feet
  • Perform personal care needs after given a patient's assignment and attending to his/her requests promptly. If unable to perform a certain task, report to the Case Manager immediately.
  • Provide patient and family with positive communication techniques, within realm of ethical and respectful care, including confidentiality.
  • Help patients dress and undress
  • Record important health information including temperature, pulse, respiration, and blood pressure
  • Assess and report patient condition and any significant changes to the Case Manager
  • Assist patients with ambulation and exercise
  • Respect patient and family/caregiver environment and patient's personal needs
  • And additional duties as assigned
  • Provide respite for patient's family/caregiver when on-site, as appropriate
  • Maintain patient's living area is clean and orderly, as assigned
  • Adhere to the organization's documentation and care procedures and standards of personal and professional conduct
  • Participate in quality assessment performance improvement teams and activities
  • Respect patient and family/caregiver environment and patient's personal needs
  • All other duties as assigned

 

Requirements: 

  • Certification of Nursing Assistant or Home Health Aide required per state regulations
  • Preferred to have one (1) year experience as Aide in community health/hospice or medical/surgical experience
  • Must have completed a minimum of 75 hours of classroom and supervised practical training
  • Licensed driver with automobile that is insured in accordance with organization/state requirements and is in good working order
  • Willingness to submit to a criminal background check prior to hire and annually thereafter
  • Current CPR certification

Knowledge, Skills & Abilities: Under the supervision of the ER/Inpatient Manager, and direction of the Charge Nurse and Registered Nurses of the unit, performs various direct patient care activities. Assists patients, across the age continuum, with activities of daily living, providing basic nursing care and assists in the maintenance of a safe and clean environment. Collects non-invasive body fluid specimens and gathers vital signs, but does not start or administer intravenous fluids. Aids physicians and nursing staff members with procedures if needed.
