Job Summary:
Resource nurses complete their primary duties and responsibilities while maintaining, modeling, and promoting the staff expectations or RJHC. They act as informal leaders on the floor, assisting with the coordination of care and helping in times of chaos.
Required Qualifications:
An unencumbered nursing license in the state, LPN/LVN or RN.
Must have, as a minimum, one (1) year of floor experience in a skilled nursing facility (SNF).
Knowledgeable of nursing and medical practices and procedures, as well as purpose of policies, procedures, laws, regulations and guidelines pertaining to long-term care.
Graduate of an accredited School of Nursing. Must compete annual continuing education required pursuant to the OARS for Oregon Nursing.
Requirements
Partners with interdisciplinary team members, physicians, consultants, and community agencies to support and improve the admissions process.
Ensures rooms are assigned and admit-ready; coordinates with central supply and maintenance on equipment and supply needs.
Delegates admission tasks (vitals, height, weight) to CNAs and coordinates follow-up.
Works with pharmacy, providers, and CMAs to ensure medications are on hand or retrieved from Cubex.
Obtains prescriptions and order clarifications from the house provider as needed.
Coordinates nursing assessments with floor nurses for late admits.
Welcomes new residents and ensures orientation, fluids, snacks, call light, and care needs are met upon arrival.
Completes head-to-toe assessment and reviews baseline care plan and medications for all new residents.
Completes and personalizes admission care plan including bathing, skin checks, ADLs, and Kardex/POC items.
Ensures skin issues are documented, care planned, and have active orders in place.
Ensures complete discharge/transfer documentation including destination, departure details, escort, supplies, medications, and any care training provided.
Confirms appropriate medications accompany residents; ensures narcotics are signed out of the narc book.
Transcribes orders, AVS, outside provider notes, and care information into the EHR per RJHC policy.
Ensures orders are second-checked and placed in RCM boxes for processing.
Maintains objective, professional documentation in the resident EHR.
Assists with audits, care planning, incident investigations, wound care, and other nursing assessments as assigned during low-volume periods.
Ability to manage the emotional and physical demands of the role, including frequent movement, resident repositioning, bending, lifting, and overhead reaching, while maintaining focus and professionalism amid interruptions and interactions with distressed residents, families, or staff.

May be exposed to infectious diseases, hazardous chemicals, and other occupational hazards inherent to a care environment.
Promotes and protects resident rights; reports any allegations of abuse, neglect, or mistreatment immediately; maintains confidentiality of all protected health information.
Complies with all facility policies, state/federal regulations, and standards of business conduct; completes all required training and competencies.
Ability to work beyond normal hours, weekends, holidays, or on-call as needed to ensure safe resident care.
Compliance with all facility standards, policies, and procedures — including the ethics and compliance program — is a condition of employment and will be considered in performance evaluations.
