What You’ll Do

• Provide skilled nursing care in accordance with physician orders and patient care plans
• Review and follow the Plan of Care during each patient visit
• Monitor patient conditions and communicate changes with the RN Case Manager and care team
• Educate patients and caregivers on disease management and self-care techniques
• Document patient care accurately and timely
• Collaborate with physicians, clinicians, and interdisciplinary care teams
• Support efforts to reduce avoidable hospitalizations and improve patient outcomes
• Deliver compassionate care and exceptional patient experience

What You’ll Need

• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
• Current Indiana LPN license
• Minimum of 1 year of nursing experience in a clinical care setting
• Home Health or Hospice experience preferred
• Reliable transportation, valid driver’s license, and current auto insurance
• Ability and willingness to travel within assigned territory
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment

Participate in orientation and in-service training for personnel. Assist physicians with special tests and procedures within the scope of state specified Nursing Practice Act.
