Position Overview

Synergy HomeCare is seeking a Registered Nurse (RN) to perform client assessments both over the phone and in person when necessary. The RN will evaluate potential and current clients to determine care needs, provide clinical insight, and ensure appropriate care plans are established for non-medical home care services.

This position is assessment-based and paid $100 per completed assessment.

Responsibilities

  • Conduct initial client assessments via phone and in person when required

  • Evaluate client health status, safety, and care needs

  • Document assessment findings accurately and professionally

  • Provide recommendations for appropriate home care services

  • Communicate findings with office staff and care coordinators

  • Maintain confidentiality and follow all HIPAA guidelines

  • Complete assessments in a timely and professional manner

Compensation

  • $100 per completed assessment

  • Assessments may be conducted virtually (phone) or in person

Requirements

  • Active Registered Nurse (RN) license in good standing (state of practice)

  • Proof of RN license required

  • Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) from an accredited nursing program

  • Minimum 1 year nursing experience preferred (home care, hospice, or geriatric experience is a plus)

  • Strong clinical assessment and communication skills

  • Ability to document clearly and accurately

  • Valid driver's license and reliable transportation for in-home assessments (if required)

Additional Qualifications (Preferred)

  • Experience in home care or home health assessments

  • Knowledge of elder care and care planning

  • Ability to work independently and manage assessment scheduling
