Position Summary:

Implements the agency program of full time professional nursing service to patients in their homes under the supervision of the Clinical Services Supervisor in accordance with agency standards and policies.

Essential Functions and Responsibilities:

  • Initiates and provides medically prescribed skilled nursing care and/or health counseling that contributes to the treatment of the medical condition, achievement of the rehabilitation potential, and/or reduction of pain and other symptoms.
  • Makes the initial visit to assess the patient’s physical and emotional status, needs, and resources through observation, interview and analysis of records.
  • Prepares the nursing plan of care, in accordance with the physician’s plan of treatment, based on problems identified in the nursing assessment and provides patient teaching using the Care Plan/Standard of Care.
  • Interprets to the patient and family the implications of the diagnosis and nature of the treatment, consistent with the Physician’s instruction.
  • Provides emotional support and encouragement to help the patient and family accept and adjust to the effects of disease, disability, or dying.
  • Supervises and evaluates patient self care or care given by others.
  • Notifies physician and Clinical Services Supervisor of changes in patient condition.

Required:

  • Graduate of an accredited school of nursing. Preference will be given to applicants with educational preparation appropriate to the immediate or long-range needs of the agency.
  • Current license to practice as a Registered Nurse in the State of Michigan.
  • Minimum of one to two years of med/surg, critical care or demonstrated experience in a specialty area.
  • Current Michigan driver’s license and proof of valid automobile insurance.

Preferred:

  • Two years hospital experience.
  • Experience in a certified home health agency.

Summary Under direct supervision of the provider, will be responsible for patient assessment and care, and maintaining appropriate documentation. This position must have basic knowledge of specialty and/or primary care and able to assess, plan, implement and evaluate care for patients.
