Nurse Navigator Capacity and Flow

Summary

Brown University Health’s CMAC is the hub of capacity management and access across the system. It serves a vital role in managing patient flow and throughput at the system level and at each individual site, having visibility across all hospital operations and maintaining communication with all disciplines and teams. The CMAC is a central department with several branches charged with managing patient flow and throughput through access, capacity and flow, and transport teams with the primary goal of ensuring the right patient is in the right place at the right time.

The Capacity and Flow Nurse Navigator reports to the Clinical Operations Manager of Capacity and Flow within the CMAC. For the Nurse Navigators defined shifts, s/he will have visibility of capacity at the system and site-level. This role will include responsibility for the coordination of all activities that involve patient placement and movement including those patients directly admitted, transferred, and discharge.

Responsibilities

Patient Placement Services:

Utilizes clinical skills to place patients and move patients (as needed) throughout the system and at the site level- identifying and expediting the most appropriate placement of admitted patients quickly and safely

Communicates regularly and collaborates with nursing operations across the system (at each site) to understand nurse staffing and patient-related issues that arise that could affect throughput and patient flow

Communicates regularly with the Patient Access team to provide current census in order to make decisions and provide direction related to transfer and direct admission acceptance.

Provides direction in moving patients within the emergency department, critical care units, acute care units and post-acute care units.

Communicates regularly with Patient Flow Expediter to understand barriers and delays and to assist with planning and prioritizing beds.

Maintains up-to-date bed availability information in the event of a community disaster

Supports and promotes the achievement of goals and standards for patient care delivery within the organization.

Operations

Communication of capacity status via e-mail, operational meetings reports, and participation at site level and system-level capacity meetings

Responds to administrative and/or operational problems as they arise, defines and resolves problems in a professional and courteous manner to multiple demands from customers.

Is capable of retrieving, entering, and updating patient records within the EMR (Epic).

Partners and collaborates with all disciplines across the hospital, sharing pertinent information in a timely fashion.

Demonstrates a high level of clinical knowledge, remains informed of current trends and innovations related to capacity management and patient flow.

Collaborates with the CMAC leadership team to provide feedback and documentation related to patient care, administrative issues, events, situations, etc.

Qualifications-education

  • Baccalaureate Degree in Nursing required.
  • Must have current licensure to practice as a Registered Nurse in the State of Rhode Island or in another state that participates in the Nurse Licensure Compact.
  • Demonstrates knowledge and skills necessary to provide care to patients throughout the life span, with consideration of aging processes, human development stages and cultural patterns in each step of the care process.

Qualifications-experience

  • Minimum of five years progressive clinical experience plus two years of administrative experience preferred.
  • Strong interpersonal/negotiation/mediation skills and demonstrated effective leadership in an environment with culturally diverse needs and abilities of staff.
  • Maintains clinical competency, in order to understand processes and identify barriers to improve the quality of patient care.
