As a Pre-Admission Coordinator, you’ll play a key role behind the scenes by reviewing referrals, coordinating admissions, and helping patients transition smoothly into home health services. What You’ll Do
✔ Review patient records prior to admission
✔ Ensure clinical documentation is complete and accurate
✔ Coordinate with referral sources, facilities, and clinical teams
✔ Track pending admissions and discharge timelines
✔ Support smooth transitions into home health services
✔ Communicate with patients, families, and healthcare partners
✔ Utilize systems such as Homecare Homebase and Workday
What You Need
✔ Active LPN/LVN license
✔ 1+ year of healthcare experience
✔ Home Health or Hospice experience preferred
✔ Strong communication and organizational skills
✔ Ability to multitask and problem solve in a fast-paced environment
✔ Basic Microsoft Office skills and ability to learn EMR systems
✔ Bilingual English/Spanish preferred
✔ Valid driver’s license and reliable transportation
