An Average Day: 

  • Ensure the educational needs of the community related to hospice care and services are assessed and met on an ongoing basis.
  • Continuously conduct audit and review of charts to ensure compliance.
  • Establish and monitor community, customer, and patient perceptions of Bristol Hospice as a high-quality provider of hospice services in the community.
  • Maintain comprehensive working knowledge in the field of community needs. Receive hospice referrals. Conference with Clinical Supervisor and/or Director of Patient Care Services to collaborate and schedule intake activity.
  • Follow intake referral processes.
  • Serve as a liaison between facility, physicians, hospitals, skilled nursing facilities, and community and hospice program with coordination of referrals/admission process and services
  • Promptly respond to customer and referral source requests and concerns.
  • Complete consent forms as assigned and indicated.
  • Submit associated documentation in accordance with administrative processes.
  •  Provide patient/family education related to hospice philosophy and care.
  • Comply with accepted professional standards of practice and principles.

Physical Environment:

  • Must have finger dexterity, arm-hand steadiness, and manual dexterity
  • Must be able to communicate with patients, families, and other health care workers • Speech clarity, speech recognition and written expression are vital abilities for performing the job.
  • Will be required to lift heavy items on a frequent basis. Must be able to lift and carry up to 50 lbs. with or without assistance and must be able to spend a prolonged amount of time standing, walking, bending, and stretching.
  • Must possess the ability to deal with stressful situations • Must be compassionate without becoming personally involved with the patients, and their families.

Requirements: 

  • Active LVN license in the state of California
  • One to two years of experience in Hospice Sales preferred.
  •  Must have excellent observation, verbal and written communication skills, problem solving skills.
  • Must be able to perform prolonged or considerable walking or standing
  • Must be able to lift supplies and equipment
  • Must be able to perform considerable reaching, stooping, bending, kneeling, and/or crouching • Must have visual acuity and hearing.
  • Must be flexible in work hours
  • Must possess a willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs

Schedule: Tuesday thru Saturday from 6:00 a.m. - 2:30 p.m. Under the direction and supervision of the Director of Nursing Services, the Treatment Nurse is responsible for all the treatments that are prescribed by the attending physician for all residents in the facility. Treatment Nurse Registered Nurse (RN) Licensed Vocational Nurse (LVN) QUALIFICATIONS • Graduate of an accredited school of professional nursing. • Current licensure as a Registered Nurse or a Licensed Vocational Nurse in the state in which practicing. • Possess one or more year’s experience in a skilled nursing care facility. • Ability to read, write and speak the English language. • Possess mental and physical health. Treatment Nurse Registered Nurse (RN) Licensed Vocational Nurse (LVN) GENERAL DUTIES AND RESPONSIBILITIES: CLINICAL • Assess resident’s skin condition by doing complete body assessment on admission, re-admission and return from day, night or extended leave of absence. • Initiate the Nurse’s Skin and Wound Progress report upon identification of any skin problem and/or pressure ulcer. • Notify attending physician promptly when any skin condition is identified. • Initiate all treatments immediately as prescribed by attending physician. • Ensure that all skin conditions have corresponding treatment orders. • Ensure that all sites of any skin problems are identified and documented on the nursing assessment form upon admission. • Photograph all newly admitted residents with any skin problems and with stage 2, 3, and 4 pressure ulcers.
