Responsibilities

Providing medical care, palliates and manages terminal illness in compliance with the state Nurse Practice Act under the supervision of a collaborating physician
Completing history and physical examinations by evaluating the patient's medical condition and health history
Completing the hospice face to face visit requirements prior to the third and subsequent hospice benefit periods to assess hospice eligibility based
Making recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care
Completing and maintaining accurate clinical notes
Communicating with the physician and participating in interdisciplinary group meetings to meet the medical and emotional needs of the patient and family/caregivers
Ensuring compliance with all state and federal legal and regulatory requirements
Additional responsibilities as assigned

Qualifications

Graduate from an accredited School of Nursing and accredited Nurse Practitioner program
Current DEA license, if applicable to state regulations
Current nursing license in state of practice
Current CPR certification
Master’s degree with minimum of one year NP experience
One year hospice or palliative care experience preferred
Accreditation from an approved certifying body for advanced practice nursing as required by state of practice
Certification in a specialist area preferred (e.g. Hospice and Palliative Nursing (CHPN) or Geriatrics)
Commitment to clinical and documentation excellence

General Responsibilities To provide medical care and treatment within established scope of practice to medical services patients as assigned by and under the direction of the collaborating physician (Section 610-X-5 of the Alabama Board of Nursing Administrative Code) or under supervision of a Board-Certified Physician (Section 540- X-7 of the Alabama Board of Medical Examiners Administrative Code). The collaborating physician shall be available for direct communication for consultation and referral or arrange for the same medical coverage by a physician who is pre-approved by the State Board of Medical Examiners in the same manner as the collaborating physician of record. To coordinate plan of care, discharge and specialty referrals as appropriate to meet the age specific needs of the patient. To plan and coordinate health care education for patient, family and other health care providers as needed. To perform comprehensive physical examinations and assessments. To interpret, analyze and evaluate patient’s medical record and social data. To perform and/or assist the physician with procedures per collaborative agreement. To coordinate and participate in performance improvement activities.
