We are seeking a dedicated and compassionate Evening Shift LPN to join our team at Complete Care at Berkshire. As an Evening Shift LPN, you will play a critical role in providing high-quality patient care in a fast-paced and dynamic environment.

About the Role: Evening Shift LPN

  • As an Evening Shift LPN, you will be responsible for providing personalized care to patients in our long-term care facility, with a focus on evening shifts.

  • You will be responsible for administering medications, monitoring vital signs, and reporting any changes or concerns to the healthcare team.

  • You will also be responsible for assisting patients with daily living activities, such as bathing, dressing, and grooming.

  • You will work closely with our interdisciplinary team to develop and implement individualized care plans for our patients.

Responsibilities: Evening Shift LPN

  • Provide direct patient care, including vital signs, wound care, and treatment administration

  • Administer medications and treatments as ordered by physicians

  • Document patient care and progress in medical records

  • Assist patients with daily living activities, such as bathing, dressing, and grooming

  • Report any changes or concerns to the healthcare team
