Responsibilities

  • Manage and provide primary care services to a designated panel of senior patients in a value-based care model
  • Drive panel performance through quality metrics (e.g., HEDIS, STARs, risk adjustment) and patient engagement initiatives
  • Coordinate care across interdisciplinary teams, including specialists, behavioral health, and care management
  • Report to the Associate Medical Director
  • Work fully onsite in Homeland Park, SC

Required Qualifications

  • Graduate of an accredited Nurse Practitioner program (MSN or DNP)
  • Current national certification (AANP or ANCC)
  • Active, unrestricted NP license in South Carolina
  • DEA license and Medicare/Medicaid credentials (or ability to obtain upon hire)

Preferred Qualifications

  • 2+ years of Nurse Practitioner experience
  • Experience managing a patient panel in a primary care or value-based care setting
  • Background in Medicare Advantage or geriatric-focused care
  • Experience with Athena EMR
  • Familiarity with HEDIS quality metrics, STAR ratings, and Medicare guidelines
