This position performs resident/patient care activities and related services necessary in caring for the personal needs, safety, and comfort of residents/patients as assigned. Assists in providing a positive physical, social, and psychological environment. The CNA II relates positively, effectively, and appropriately with residents/patients, families, community members, volunteers and other facility staff. Successful incumbents have fast and adaptive problem-solving skills who value collaboration and teamwork. This position has an in-depth understanding of patient care policies and regulations and is comfortable working with specific populations of patients, including elderly patients and those with special needs.

 

Essential Functions and Responsibilities:

1. Assists with resident/patient care needs and comforts including but not limited to:

  • Assists or gives oral hygiene and mouth care.
  • Assists with bathing functions and dressing/undressing as necessary.
  • Assists with nail care, shaving and hair care.
  • Assists with lifting, turning, positioning and transporting residents.
  • Makes both occupied and unoccupied bed (as assigned).
  • Keeps residents/patients dry (i.e. change gown, clothing, linen, etc.) when they become soiled or wet.
  • Assists resident/patient with bowel or bladder functions.
  • Checks resident/patients frequently to assure that their personal care needs are being met.

2. Assists with nursing functions as follows:

  • Measures and records vital signs.
  • Weighs and measures residents/patients.
  • Collects Specimens (i.e. urine, stool, sputum)
  • Prepares and gives enemas.
  • Provides daily indwelling catheter care.
  • Performs diabetic urine testing.
  • Turns bedfast residents/patients at least every two hours.
  • Provides daily range of motion exercises.
  • Assists with care of dying residents.
  • Maintains accurate intake and output records.
  • Noting observations of patient behavior, including complaints, or physical symptoms to nurses.

3. Performs food service functions including:

  • Prepares resident/patients for meals (i.e. take to bathroom, wash hands, oral care, take to and from dining room, etc.).
  • Serves food trays. Assists with feeding as indicated.
  • Records the residents/patients food fluid intake.
  • Serves between meal and bedtime snacks.
  • Keeps resident’s/patient’s water pitchers clean and filled with fresh water on each shift.
  • Performs after meal care (i.e. clean residents/patients hands, face, clothing, brush teeth, take to bathroom, etc.).
  • Notes and reports changes in resident/patient eating habits, especially those on diabetic diet.
  • Assists with keeping nursing and resident/patient care area neat, sanitary and orderly.
  • Floats to the OVCH Acute Care Facility four (4) times each year.
  • Appropriately records and charges for supplies.
  • Performs other duties as assigned.

Qualifications

Required: 

  • Current Certified Nursing Assistant (CNA) certification.
  • Current Basic Life Support (BLS) certification.

Preferred:

  • High school Diploma or equivalent (GED).
  • Six (6) months of CNA experience performing Nursing/Patient Care duties in an acute care setting or skilled nursing facility.

Report missing ID bands to admission coordinator immediately. • Make beds, clean bedside and overbed tables. • Serve drinking water as required. Assure that water pitchers are cleaned, filled with fresh drinking water and within the resident’s reach. Pass water pitcher as per established policy and procedure. • Dress residents neatly and in their own clothing. Assist residents in dressing as necessary. • Assist in preparing residents for meals (taking to/from dining room, serving trays, placing bibs, assisting in feeding or cutting food, removal of trays, supervision in dining room, etc.). • Serve nourishment in accordance with established facility procedures. • Feed residents who cannot feed themselves. • Assure that resident’s food is accessible and self-help devices are available as needed. • Prepare residents for therapy, activities, physician visits, religious services, etc., to include transporting residents to/from such activities. • Keep incontinent residents clean and dry as possible at all times. Change bed linens, diapers and clothing as often as necessary. • Do not permit residents to smoke in bed. • Assure that the nurse’s call system is attached to the bed and within easy reach at all times when residents are bedfast. • Answer residents’ call lights promptly. • Check for food in residents’ room. Ensure that food is kept in a sealed container. • Report all complaints to the Charge Nurse. • Report any resident abuse immediately (ie., harsh/abusive language, unnecessary roughness, etc.) • Report any medication found in the resident’s room. Take to the Charge Nurse immediately. • Report all hazardous conditions and faulty equipment to the Charge Nurse immediately. • Report any bruise, skin tears, incidents or accidents to the Charge Nurse immediately. • Attend in-service educational programs, on the job training programs and meetings as directed. • Other related duties as may become necessary or as directed by the Charge Nurse, RN Supervisor, Director of Nurses, or Administrator. Certified nursing assistant (CNA) CLINICAL • Wash hands before and after caring for each resident. • Place residents in correct and comfortable position. Head of bed elevated at 30 for all residents on enteral feeding. • Assist residents with dental and mouth care. Clean dentures after each meal. Keep dentures clean and soaked with Efferdent after dinner. • Record resident’s food and nourishment intake as directed. • Perform after meal care as required – cleaning resident’s hands, face, clothing, dentures, etc. • Assist residents to/from bathroom. Offer bedfast residents bedpans and urinals. Clean and store such equipment after each use (document Intake and Output as directed). • Check and document bowel movements and character of stools every shift. • Bathe residents as assigned and in accordance with established facility procedures (encourage showers and other self-help measures/activities). • Comb and brush resident’s hair daily. Shampoo hair on shower days. • Clip and trim fingernails. • Shave male residents daily or as needed. • Keep hair under arms and face on female residents clean-shaven as necessary or instructed. Trim hair in nose and ears as needed. • Observe and report the presence of reddened areas on bony prominences. • Keep drainage bag off the floor when resident is in bed or in wheelchair. • Apply Vaseline or A & D ointment to dry skin in accordance with established policies. • Move and turn bedfast residents at least every two hours and record such information on CNA notes. • Observe disoriented and comatose residents and notify Charge Nurse immediately. • Apply postural support properly. Check with Charge Nurse for type of restraint to be used. • Apply hand roll or splint to residents with contractures as outlined within the facility’s established policies and procedures. • Follow infection control procedures in accordance with established facility policy when isolation techniques are necessary. • Chart required information every shift. • Complete Intake and Output documentation on all residents with orders before the end of each shift. • Other related duties as may become necessary or as directed by the Charge Nurse, RN Supervisor, Director of Nurses, or Administrator. FTE: 0.500000 Standard Hours: 20.00 Shift: Shift 3 Shift Details: 2145 to 0745. 2 days/week Holidays : Varies based on the work schedule Weekends : Every other weekend required Job Summary: The Certified Nursing Assistant (CNA), under the direction and supervision of a Registered Nurse, provides delegated age appropriate care in a culturally and ethnically sensitive manner. The CNA assists patients with the tasks outlined in their clinical competency assessment related to basic nursing skills, personal care skills and basic restorative services. The CNA practices and performs what was taught during their CNA training and certification or learned during basic nursing courses at an accredited school for professional nurses. The CNA may be asked to provide continuous safety observation. This job reports to the Nurse Manager/Assistant Nurse Manager of the assigned unit and the RNs on duty on each assigned shift. Other duties as assigned. EDUCATION DESCRIPTION: High School diploma or equivalent is preferred. LICENSURE DESCRIPTION: State of Wisconsin Nursing Assistant Certification is required, otherwise must be currently enrolled in a nursing program and able to provide documentation from an accredited school for professional nurses indicating successful completion of the basic nursing course which encompass all of the criteria noted by the State of Wisconsin administrative code and a minimum experience of one acute care medical/surgical clinical rotation. American Heart Association (AHA) Basic Life Support for the Healthcare Provider (BLS) or an AHA approved equivalent is required within 90 days of hire.
