Job Summary

The Discharge Nurse Navigator (DNN) as part of the Discharge Navigator Nurse team, utilizes evidence-based approaches to the hospital discharge process, assisting patients/families navigating their hospitalization course to assure a safe transition of care to home. The DNN conducts daily rounding on inpatients, proactively identifying potential barriers/gaps to discharge, collaborating and following up with the appropriate members of the healthcare team, and assist in optimizing each patients’ discharge planning. The DNN reinforces education of diagnosis, plan of care, medications, home care needs, etc., utilizing teach-back technique in all aspects of care from admission throughout their admission to time of discharge. As part of the DNN team, the DNN actively participates in quality improvement activities to improve processes and outcomes as related to assuring a safe transition home for CHOC’s patients and families.

Pay Range

Minimum $59.81 Midpoint $77.75 Maximum $95.69

Rate of pay is determined by various factors, including but not limited to knowledge, skills, competencies, experience, education, as well as position requirements.

Experience

• Required: Minimum two (2) years of inpatient pediatric experience.

• Preferred: Five (5) + years of nursing experience, two (2) + years of inpatient pediatric experience.

Education

• Required: Bachelor of Science degree in Nursing from an accredited school.

• Preferred: Master of Nursing degree.

Training
