Elevate Care is looking for a dedicated Infection Preventionist RN or LPN to lead our infection control efforts and support our mission of providing a safe, healthy environment for both residents and staff. If you're passionate about clinical excellence and regulatory compliance, we want to hear from you.

Key Responsibilities:

  • Develop, implement, and monitor the facility’s infection prevention and control program

  • Perform ongoing surveillance and investigate any outbreaks or infection trends

  • Provide training and education to staff on infection prevention best practices

  • Ensure compliance with CDC, CMS, OSHA, IDPH, and other applicable guidelines

  • Collaborate with clinical teams to reduce healthcare-associated infections (HAIs)

  • Conduct audits and maintain documentation for state and federal survey readiness

Requirements:

  • Current Illinois RN or LPN license in good standing

  • Experience in infection prevention/control preferred

  • Certification in Infection Control (CIC) is a plus or willingness to obtain

  • Familiarity with long-term care or skilled nursing facilities is strongly preferred

  • Excellent organizational and communication skills
