Nurse Practitioner – – Internal Medicine - Broadway Practice - ACN/Columbia Campus

The Broadway Practice has transformed how we provide care to the Washington Heights community through its successful pilot of the Interdisciplinary Care Team, core to the Patient-Centered Medical Home model. All members of the care team—Nurses, Nutritionist, Care Manager, Community Health Worker, Patient Financial Assistants, Social Worker, Medical Assistants, and Physicians collaborate to improve the health and well-being of our patients.

This is a full time, Monday through Friday position – 8:30 AM to 5:00 PM

Preferred Criteria:

Minimum three years of experience as a Nurse Practitioner in Internal Medicine
Out-patient experience
Bilingual - English/Spanish
