We’re hiring a Nurse Practitioner – a compassionate and skilled clinician who provides advanced nursing care, supports patients and families through end-of-life journeys, and ensures compliance with regulatory standards. This role is essential to delivering dignified, patient-centered care and supporting the smooth operation of our hospice program.

What You’ll Be Doing

  • Perform comprehensive nursing assessments for hospice patients
  • Assess signs and symptoms of discomfort, including pain, and take prompt, appropriate action
  • Initiate and manage individualized plans of care for patients and families
  • Provide direct face-to-face visits for referred and existing patients
  • Communicate with physicians regarding patient updates and care plans
  • Maintain knowledge of state and federal regulations and JCAHO standards (if applicable)
  • Participate in Quality Assessment and Performance Improvement (QAPI) programs
  • Ensure all services comply with hospice standards and regulatory requirements

Qualifications

  • Graduate of an accredited school of Advanced Practice Registered Nursing
  • MSN or Doctoral Degree required
  • Current Advanced Practice Registered Nurse License in the state of employment
  • End-of-Life Nursing Education Program (ELNEC) – all 8 modules completed within 18 months of hire
  • Certification in Hospice and Palliative Nursing preferred
  • Valid driver’s license, good driving record, and reliable transportation
  • Ability to lift up to 50 pounds
  • Must clear criminal background check, physical, and drug screening
  • Comfortable working outside regular hours as needed
