Legacy is recruiting an Outpatient Advanced Practitioner with 3+ years of primary care experience for our Canby Clinic located at 1433 SE 1ST AVE., Canby, OR.  Providers in this clinic include physicians and advanced practitioners. Our primary care providers are supported by individually assigned MAs as well as care teams that can include panel coordinators, social workers, RN case managers, behavioral health clinicians and a pharmacist.

 

Our clinics support the local communities by providing services to diverse patient populations. With more than 25 clinics across the Portland metro area and rural locations such as Silverton, Keizer, Sandy and St. Helens, Legacy is helping keep the Pacific Northwest healthy.

Responsibilities

  • Responsible for the patient throughout the course of treatment, including reviewing clinical history, providing the physical examination, making the diagnosis, and treating both chronic and episodic disorders.
  • Work in collaboration with the team and seek the advice of a team physician whenever a case falls outside the scope of expertise.
  • Maintain own panel of patients and share call coverage for the clinic.

Qualifications

  • Graduate of NP or PA program.
  • Minimum of 2-years’ experience.

Licensure

  • Current Oregon State license.
  • Current BLS certification from the American Heart Association.
