Legacy is recruiting an Outpatient Advanced Practitioner with 3+ years of primary care experience for our Canby Clinic located at 1433 SE 1ST AVE., Canby, OR.  Providers in this clinic include physicians and advanced practitioners. Our primary care providers are supported by individually assigned MAs as well as care teams that can include panel coordinators, social workers, RN case managers, behavioral health clinicians and a pharmacist.

 

Our clinics support the local communities by providing services to diverse patient populations. With more than 25 clinics across the Portland metro area and rural locations such as Silverton, Keizer, Sandy and St. Helens, Legacy is helping keep the Pacific Northwest healthy.

Responsibilities

  • Responsible for the patient throughout the course of treatment, including reviewing clinical history, providing the physical examination, making the diagnosis, and treating both chronic and episodic disorders.
  • Work in collaboration with the team and seek the advice of a team physician whenever a case falls outside the scope of expertise.
  • Maintain own panel of patients and share call coverage for the clinic.

Qualifications

  • Graduate of NP or PA program.
  • Minimum of 2-years’ experience.

Licensure

  • Current Oregon State license.
  • Current BLS certification from the American Heart Association.

Essential Duties and Responsibilities: • Participate in direct patient care, which may include family planning, newborn care, pediatrics, and general adult care. • Meet minimum patient care productivity requirements as may be set by the Center to meet requirements of outside funding and/or reimbursement sources. • Provide continuous, comprehensive, and culturally sensitive health care of the highest quality. • Provide all necessary documentation in the medical record of care provided to patient, including education, medications, referrals, diagnostic testing, and follow-up. • May assist in organizing and participating in special clinics such as the migrant camp program. • Provide day-to day clinical supervision of support staff with respect to individual patient management. • Participate in case management review and discussion as needed. • Assist in implementation of plans to meet the objectives of the annual “Health Plan” and other goals established by the QA/QI committee and the Center’s managed care plans. • Participate in chart audits to assess success of QA/QI program. • Participate in medical staff, OB, QA/QI meetings and other committees within the Center established to improve patient care and the overall operation of the Center. • Provide supervision of students and residents in the health care professions. • Perform other duties as assigned. • Handle protected health information (PHI) in a manner consistent with the Health Insurance Portability and Accountability Act of 1996 (HIPAA). HIPAA Requirements: The Nurse Practitioner/Physician Assistant will have access to PHI in the course of carrying out their duties.
