Qualifications

A heart to serve patients and families and a passion for providing the best possible care
Education: Graduate of an accredited school of nursing with a current state license as a licensed practical nurse
Experience: 2+ years of nursing experience in an acute care setting, or equivalent experience (hospice experience preferred)
Required: Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 80-100lbs and bear the weight of an average adult effectively.

Parham Healthcare & Rehabilitation Center is seeking a full time MDS Coordinator with a license to practice as a Registered Nurse for our 180 bed skilled nursing facility in Richmond. The typical schedule for the MDS Coordinator is Monday-Friday with a weekend rotation of Manager on Duty. The MDS Coordinator is responsible for completing minimum data set assessments and creating comprehensive plans of care after review of the patient's medical record and communication with direct care staff, the patient's physician, and family.
