Per Diem Registered Nurse (RN) – VA & Medicaid Assessments
A Place At Home is seeking a compassionate and experienced Per Diem Registered Nurse (RN) to conduct Veteran Affairs (VA) and Medicaid client assessments and support the delivery of high-quality, person-centered home care services. This role is responsible for performing comprehensive nursing assessments, developing and updating plans of care, identifying client needs, and ensuring compliance with Colorado nursing standards, Medicaid requirements, VA program expectations, and agency policies.
The ideal candidate is an independent, detail-oriented clinician who is passionate about advocating for Veterans and seniors while ensuring safe, effective, and compliant care.
Essential Responsibilities
VA and Medicaid Assessments
Complete initial nursing assessments for new VA clients within required program timelines.
Conduct reassessments for VA clients at least every 60 days, or more frequently as clinically indicated.
Perform Medicaid assessments and reassessments in accordance with state regulations and agency requirements.
Collect and evaluate health history, medication information, functional status, psychosocial factors, and environmental safety concerns.
Assess activities of daily living (ADLs), instrumental activities of daily living (IADLs), cognitive status, fall risk, pain, and overall health needs.
Clinical Assessment and Care Planning
Identify actual and potential health problems and care needs through comprehensive nursing assessment.
Develop, implement, and update individualized nursing care plans based on assessment findings.
Monitor changes in client condition and communicate significant findings to the appropriate provider, case manager, and interdisciplinary team.
Evaluate the effectiveness of services and recommend modifications to the plan of care as needed.
Advocate for appropriate home care services and support based on clinical findings and client needs.
Documentation and Compliance
Maintain accurate, timely, and complete clinical documentation.
Ensure all assessments, care plans, and related records comply with Colorado nursing standards, Medicaid requirements, VA program expectations, agency policies, and applicable accreditation standards.
Submit required documentation within established timelines.
Participate in quality assurance and compliance activities as requested.
Communication and Coordination
Collaborate with caregivers, home health aides, providers, case managers, and other members of the interdisciplinary care team.
Communicate effectively with clients and family members regarding assessment findings, care needs, and available services.
Serve as a clinical resource and advocate for Veterans, seniors, and their families.
Qualifications
Current, unrestricted Registered Nurse (RN) license in the State of Colorado.
Minimum one (1) year of nursing experience; experience in geriatrics, home care, community health, care management, or long-term care preferred.
Knowledge of comprehensive nursing assessment, care planning, and clinical documentation practices.
Ability to exercise independent nursing judgment consistent with Colorado RN scope of practice.
Strong assessment, communication, organization, and time-management skills.
Valid driver's license, reliable transportation, and ability to travel throughout the service area.
