Job Description

Santa Barbara Cottage Hospital seeks a Utilization Management Case Manager for their Care Management department responsible for the utilization management, quality assurance, and discharge planning activities for assigned services/areas/patients within Cottage Health. Case management activities will result in quality outcomes, optimal care/cost management of services and/or procedures, a high level of customer satisfaction and contribution to an overall value-oriented experience of stakeholders and persons served.

Education:

  • Minimum: Associate's Degree in Nursing (ADN).
  • Preferred: Bachelor's Degree in Nursing (BSN).

Certifications, Licenses, Registrations:

  • Minimum: Current California Nursing license in good standing.
  • Preferred: Certification in Case Management.

Years of Related Work Experience:

  • Minimum: 2 years direct patient care experience in an acute care setting. Other patient care experience may be considered.
  • Preferred: Previous experience as a case manager in an acute care setting.

Performs ongoing, systematic collection and analysis of patient data pre – during – post hemodialysis treatment for assigned patients and documents in the patient medical record, makes adjustments or modifications to treatment plan as indicated and notifies Team Leader, Charge Nurse, Supervisor or Physician as needed. Assesses, collaborates, and documents patient/family’s basic learning needs to provide initial and ongoing education to patients and family. Directs and provides, in collaboration with direct and ancillary patient care staff, all aspects of the daily provision of safe and effective delivery of hemodialysis therapy to assigned patients.
