Responsibilities

As Branch Director, you supervise the clinical operations of your team of field staff and report directly to the Agency Administrator. You will need to be organized, detail-oriented, and possess effective communication skills both orally and in writing. You will need to understand the issues related to the delivery of home health care services and be able to problem solve. Previous home health and management experience will serve you well in this rewarding role, and an excellent knowledge of OASIS and Medicare guidelines is most helpful as you will be reviewing plans of care on a daily basis.  

Qualifications

  • Currently licensed as a Registered Nurse in the state in which the position is based.
  • Must be a graduate of an approved school of professional nursing.
  • He/She must have a minimum of one-year nursing experience within the last 36 months.
  • Medicare home health experience is strongly preferred.  
  • Previous management/supervisory experience is preferred.
    • Requirements:      1. Must have a current driver's license and automobile insurance.

The Registered Nurse RN consults as needed with the physician and the office giving details about patient care. Performing accurate OASIS collection, ensuring the medication profile remains current, ensuring lab values have been reported to the physician timely, attending weekly case conference and monthly case manager meetings are key responsibilities of this role. This part-time salaried position works 24 hours/week, over 4 days in the week, and is eligible for our 60% paid days off program.     Description The registered nurse provides home visits to women and their families enrolled in the Nurse Family Partnership Program. The RN will carry a caseload of up to 25 clients, utilizing nursing processes and clinical expertise to build therapeutic relationships, engage in motivational interviewing and mutual goal setting to assist high risk mothers in developing healthy plans for their families. The RN is accountable for the delivery of safe, quality care that is individualized for their assigned clients and follows the nursing process in accordance with the Nurse Practice Act (assessment, planning implementation, evaluation).
