Minimum Qualifications: 

1. Bachelor’s degree in Nursing (BSN).

23. Selects unit staff and ensures that all members are competent to fulfill their assigned responsibilities.  

24. Responsible for patient and family satisfaction. 

25. Manages systems in order to accomplish patient, unit, departmental and institutional goals.  

26. Facilitates and promotes achievement of unit based/practice area goals in line with the medical center mission. 

27. Assures standards of care are consistently achieved on the unit/practice area. 

28. Seeks opportunities to build working alliances with other professional groups. 

29. Develops and monitors systems that promote and support nursing practice.

Physical Requirements:  

1. Prolonged, extensive, or considerable standing/walking. 

2. Lifts, positions, pushes and/or transfer patients and equipment. 

3. Considerable reaching, stooping, bending, kneeling, crouching. 

4. Frequent exposure to hazardous chemicals, sick patients, bodily substances, noise and possible exposure to radiation, lasers, electric shock, etc. 

5. Regularly exposed to the risk of bloodborne diseases and other transmissible infections. 

6. Requires manual dexterity using fine hand manipulation to reports and computer equipment. 

7. Hearing and visual acuity sufficient to perform examinations, observe patients, read monitors and documents, and hear audible equipment alarms. 

8. May have contact with hazardous materials.

ESSENTIAL FUNCTIONS: Provide outreach services to children and families Consult with team members in the development of individualized plans and multidisciplinary services for assigned clients Maintain client records completely and accurately to include IFSP’s with full assessment of client functioning and goals, documentation of client progress in the program, records of required contact with collaterals and family members, documentation of arena assessments and documentation of DPH forms and regulations regarding intake and discharge of clients Complete billing documentation according to schedule established by the company Adhere to professional standards and guidelines according to their respective disciplines Keep supervisor informed of progress of all clients Meet direct service requirements based on part time or full time status, as designated Adhere to clinical recommendations from clinical administration Maintain confidentiality in all clinical matters Remain compliant with billing and documentation in accordance with direct service requirement Conduct visits with clients and relevant collaterals in natural environments according to the needs of the child and family Develop group curricula for assigned groups as applicable. Participate in team meetings, supervision and in-service training as required Participate in program development and team goals Obtain necessary certifications as required (i.e. DEEC, EI certifications, etc.) and maintain licensure as applicable.
