Overview

All Ways Caring Homecare is seeking a Part Time or PRN Registered Nurse to provide direct and indirect care to clients in the comfort of their home environment.  

 

The nurse supervises and directs the efforts of the Caregiver when involved in the client’s plan of care. Based on state regulations, nurses may perform administrative and/or field/clinical responsibilities. 

External Job Description

  • Provides skilled nursing care/treatment to clients per Physicians Orders and Plan of Care 
  • Documents nursing actions of all care given to individuals and informs physicians and family accordingly 
  • Administers and monitors medication and management as prescribed by physician 
  • Conducts nursing assessment visit as needed 
  • Follows policies and procedures according to operation, state and regulatory standards 
  • Completes supervisory visits based on compliance and regulations to include review of plan of care, caregiver competency assessment, and completion of required documentation
  • Implements nursing support for individuals in a home setting 
  • Works with Director or Nursing/Nurse Supervisor and/or Branch Manager to address the health needs of individuals 
  • Participates in staff meetings, in-services and other meetings as needed 
  • Ensures compliance with physician’s verbal orders and medical reports 

Qualifications

  • Degree in Nursing from an accredited school 
  • One or more years of nursing experience preferred 
  • Current and unrestricted license in the state of the operation 
  • CPR professional certification 
  • Excellent communication skills with all levels of personnel, internal and external to the company 
  • Capable of working independently with highly confidential information

Accurately administers medications, per ordered ,utilizing at least two client identifiers. Notifies providers of significant changes in status such as difficulties with detox, medical emergencies, pertinent lab results, or change in medical status regarding response to medications, etc. Assesses and documents effects of PRN medications. Maintains accountability of administration, recording, control and security of medications, including narcotics. Completes admission and discharge processes efficiently and accurately as directed. Confers regularly with treatment team (e.g., nurses, therapists, program managers, provider) regarding client care. Attends and participates in the unit activities such as therapy groups, family meetings as appropriate, staffing, unit meetings, etc. Provides education/explanation to clients, families, and co-workers regarding medication usage/maintenance, common side effects, client's rights, as well as other topics necessary for optimal client care. Monitors client acuity, census, staffing needs, and escalates coverage needs to supervisor/Charge RN. Ensures thorough, accurate documentation is completed for all clients on each shift and client assignment sheets are completed daily and tasks are carried out as assigned. Communicates thorough hand-off reports to oncoming staff and supervisors. EDUCATION/EXPERIENCE/SKILL REQUIREMENTS: Graduation from an approved/accredited school of nursing. One or more years’ nursing experience within clinical area of service provided by facility preferred. Experience in behavioral health/psychiatric/addiction recovery environment preferred. Knowledge of and experience with psychotropic medications and medication administration procedures. Ability to apply nursing standards and practice within an acute psychiatric setting LICENSES/DESIGNATIONS/CERTIFICATIONS: Valid current RN license from the state or from a compact state, where applicable. Certified in Psychiatric Nursing preferred. CPR and de-escalation and restraint certification required (training available upon hire and offered by facility). First aid may be required based on state or facility requirements. WORK LOCATION: This position is onsite at the facility and is not a remote position.
