What You’ll Do:

Provide nursing care to ensure resident needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines

Administer medications and treatments with precision and compassion

· Collaborate with RNs, CNAs, and interdisciplinary teams

· Support residents’ physical, emotional, and psychological well-being

· Assist with resident comfort.

· Collect samples and monitor catheters. Provide wound care.

· Keep resident patient information confidential, follow all policies and procedures, and adhere to federal, state, local requirements, and professional standards.

· Collect, track, maintain, and review patient medical history, charts, and physical condition, including heart rate, blood pressure, temperature, and respiration.

· Help coach staff as necessary.

Requirements:

Current LPN license in [State]
CPR/BLS certification (or willing to obtain)
Previous LTC or SNF experience preferred, but new grads welcome!
Positive attitude, reliability, and strong clinical judgment

Description Job Description PeaceHealth is seeking an LPN Clinic - Orthopedics for a Full Time, 1.00 FTE, Day position. The salary range for this job opening at PeaceHealth is $27.38 – $41.06. The hiring rate is dependent upon several factors, including but not limited to education, training, work experience, terms of any applicable collective bargaining agreement, seniority, etc. Hiring Bonus may be available! Responsible for ensuring delivery of quality nursing care to clinic patients as directed by physicians and other healthcare providers. The Clinic LPN functions as a core member of the interdisciplinary care team and fulfills/shares the roles and responsibilities of rooming nurse, resource nurse, and clinical support nurse in the ambulatory patient care setting. Essential Functions Assesses and implements patient care under supervision. Prepares patient for examination. Provides direct patient care. Assists provider with examination and treatment of patient as directed. Under the oversight of provider, may be responsible for telephone triage using age specific triage protocol books and computerized resources. Documents patient care in health record according to department standards. Obtains and conveys information to and from patients, pharmacies, and other providers as directed and supervised by a licensed provider. May obtain referral authorizations for treatment as directed by provider. Ensures own professional effectiveness through continued education and professional development. Qualifications Education Accredited State Board of Nursing LPN Program Required: Experience Minimum of 1 year Preferred: clinical experience as an LPN and Required: Demonstrated knowledge of and ability to apply age specific principles of growth and development and life stages to meet each patient’s needs Credentials Required: Upon Hire Licensed Practical Nurse - Washington Required: Upon Hire Basic Life Support Skills Basic computer skills required including familiarity with MS Office applications.
