Job Summary

Unity Health and Rehabilitation Center is seeking compassionate and dedicated LPNs to join our part-time or per diem team. Responsibilities

Provide high-quality patient care, including administering medications, wound care, and other treatments as prescribed by physicians
Develop and implement individualized care plans, working closely with the interdisciplinary team to ensure comprehensive care
Monitor patient progress, reporting changes and concerns to the healthcare team
Maintains accurate and detailed records of patient care and treatments
Participates in team meetings and contributes to quality improvement initiatives
Collaborates with other departments, including therapy and rehabilitation staff, to ensure a holistic approach to patient care
