Under the direction of the unit director, charge nurse and registered nurse, the LPN is responsible for the care and safety of the patient.

Potential Shifts:

Monday - Saturday 8:00 AM - 8:00 PM

Sunday 8:00 AM - 5:00 PM

Completion and Proof of 30 hour Florida State Approved IV
Florida Practical Nurse License (LPN)
Basic Life Skills certification (BLS)

Participate in call schedules and provide on-call nursing support as directed. About You Qualifications Licensed Practical/Vocational Nurse (LPN/LVN) licensed in the state of employment. CPR certification required. Valid driver’s license and current automobile insurance. Graduate of an accredited Practical Nursing program. Minimum of 2 years’ experience as an LPN/LVN in hospice, home health, or long-term care; 3–5 years preferred. Work Environment Care is provided in patient homes or institutional settings with travel required. Exposure to infectious diseases, bodily fluids, and hazardous materials possible. Skills and Knowledge Strong knowledge of hospice patient needs, terminal illness care, and interdisciplinary team roles. Excellent patient assessment and communication skills. Understanding of regulatory guidelines such as Medicare, Medicaid, and ACHC. Ability to provide compassionate support to patients and families during end-of-life care.
