About the Role:

As a Home Health Aide/PCA, you will play a vital role in providing high-quality care to our clients in the comfort of their own homes. Your responsibilities will include:

• Assisting with daily living activities, such as bathing, dressing, and grooming
• Managing medication and vital signs
• Providing mobility and transferring assistance
• Maintaining a safe and clean living environment
• Developing and implementing individualized care plans
• Communicating effectively with clients, families, and healthcare professionals

Responsibilities:

• Deliver exceptional care and support to clients in their homes
• Perform tasks required for daily living activities, such as personal care, meal preparation, and light housekeeping
• Assist with medication management, vital sign monitoring, and basic wound care
• Collaborate with healthcare professionals to develop and implement individualized care plans
• Communicate effectively with clients, families, and healthcare professionals to ensure continuity of care
• Participate in ongoing education and training to enhance skills and knowledge
