This job is an integral part of the care delivery system working together to provide a healing environment, by placing our customers first. The incumbent provides primary healthcare services for non-acute or chronic health conditions in specific populations in accordance with applicable scope and standards of practice and with the policies, values, and mission of the organization within the CRNP scope of practice. Work is performed in a physician office or offices dedicated to Primary Care, weekend, and on-call coverage on a rotational basis, as required.

Essential Responsibilities

  • Provides appropriate patient care to include recording patient medical history and physical assessment, developing therapeutic care plan, ordering diagnostic tests, interpreting test results, providing accurate medical record documentation. 25%
  • Communicates and coordinates patient care with patient, family, community health services and interdisciplinary team. Involves attending physician regarding plan of care and involves physician in clinical decision making of any issues that are outside the CRNP's knowledge base or scope of practice. 15%
  • Provides monitoring and continuity of care and coordinates periodic evaluation by physicians as defined in the written agreement between the CRNP and the supervising physician(s).10%
  • Prepares written prescription orders for drugs and controlled substances, administers medication and injections, sutures wounds and provides wound care10%
  • Educates patients and family members regarding health and preventive medicine, medications prescribed, treatment instructions and recommends community resources as appropriate.10%
  • Triages patient telephone calls, provides monitoring and continuity of care10%
  • Participates, develops, and implements performance and/or quality improvement initiatives. Ensures compliance with all regulatory agencies.10%

  • Master's degree MSN from the practicing State Board of Nursing Approved Master's Program
  • National Board Certification for Nurse Practitioners through an accredited association
  • Practicing-state licensure as a Nurse Practitioner
  • Prescriptive Authority from the practicing state Within 60 days of hire
  • Drug Enforcement Agency Licensure Within 60 days of hire

Additional Job Description: WVUHS Medical Group seeks a GI APP. The successful candidate will practice inpatient GI in Weirton, WV. POSITION SUMMARY : The Advanced Practice Professional (APP) has coordination and clinical skills required for the comprehensive management of gastroenterology patients, (including biological, social, and psychological assessments). The APP coordinates involvement of medical specialty teams necessary to develop a comprehensive plan for the patient and reviews the plan of care with the other members of the clinical team, participates in directing that care, performs diagnostic/therapeutic procedures and coordinates disposition decisions and discharge planning/follow-up (as applicable). The APP participates in the education of residents and medical/nursing students. This is a 5-day-a-week position. Provide healthcare services consistent with state law and the requirements of the professional licensing or certification authority and/or hospital privileges, where applicable. Such service may include but are not limited to. MINIMUM QUALIFICATIONS FOR NURSE PRACTITIONER : EDUCATION, CERTIFICATION, AND/OR LICENSURE: 1. Graduate of a CCNE accredited nursing program with Master’s Degree in Nursing 2. National Certification by one of the recognized APRN Certification Centers 3. Consults and communicates with clinic and hospital staff regarding abnormal findings in the evaluations of patients. 7. Performs succinct comprehensive clinical evaluations and documents findings in medical record. 8. Facilitates patient disposition by coordinating appropriate services and tests for follow-up appointments in the clinic (as applicable) or location of necessary care arrangements. 9. Develops a comprehensive individualized plan of care for gastroenterology/hepatology patients, which includes teaching, disposition planning and necessary referral to other members of the health care team as necessary. 10. Demonstrates expertise in caring for patients across the age continuum of their educational experience and certification. 11. Identifies unstable or complex gastroenterology/hepatology patient care situations and facilitates intervention if appropriate in the clinic setting. 12. Recognizes emergency situations and initiates effective emergency care. 13. Evaluates gastroenterology/hepatology patients and serves as a coordinator for follow up in the ambulatory setting or longitudinal care facility (as applicable). 14. Assists in the collection of outcome measures (as applicable). 15. Demonstrates critical thinking and diagnostic reasoning skills in clinical decision making by the identification, evaluation and management of the individual gastroenterology/hepatology needs of patients/families. 16. Demonstrates the ability to utilize equipment in a safe manner. 17. Bases decisions made/actions taken on the relevant scientific principles, established standards of care and/or research findings. 18. Utilizes teaching/learning theory and appropriate teaching strategies in the provision of education to nursing staff, hospital staff, students, patients, families and the community. 19. Promotes an environment that facilitates learning by responding to appropriate patient/family preferences, priorities, and readiness to learn. 20. Updates own knowledge base throughout the year. 21. Completes all identified clinical competencies. 22. Serves as a resource person for staff caring for the gastroenterology/hepatology patient. 23. Provides educational programs to professional and paraprofessional personnel in area of expertise. 24. Participates in community programs, distribution of literature, as appropriate in area of expertise. 25. Documents the plan of care, interventions, evaluations, and disposition plans in a clear and concise manner for every patient visit in the patient’s medical record following approved hospital/UHA policies and compliance standards for documentation. 26. Collaborates with other leaders and staff to effectively achieve patient outcome-based improvements in gastroenterology/hepatology. 27. Provides leadership in the development and implantation of changes in the gastroenterology/hepatology practice that positively impact patient outcomes. 28. Works collaboratively with medical and nursing staff to facilitate creative problem solving and provide professional support. 29. Participates and initiates inter-disciplinary approaches to patient care, program development and education. Serves as a professional role model. 30. Promotes and practices innovation of the expanded role in the delivery of care to gastroenterology/hepatology patients and families throughout the inpatient hospitalization, and the ambulatory setting and/or longitudinal facility. 31. Participates and provides leadership for committees and task forces. 32. Documents hours and submits billing reports, as appropriate. 33. Reviews and interprets hepatology data from sources such as patient registries, clinical and financial reports, length of stay, access to care, and cost per case reports and recommends/implements changes as necessary. 34. Participates in clinical trials and research studies with the Principal Investigator. 35. Assists hepatology Faculty, Administrators and Directors in the collection of outcome measures. 36.
