Job Responsibilities

Provide holistic nursing care to individuals with intellectual and developmental disabilities
Assist the Registered Nurse in delivering quality nursing services to persons supported
Provide health related training to unlicensed personnel
Administer medications and treatments or assist the person supported with medication administration
Assist with ensuring preventative and routine medical appointments are scheduled
Conduct medication closest assessments
Collect health related data
Observe for and report any abnormal health related findings to the physician
Communicate with physicians, pharmacies, medical suppliers, and the Interdisciplinary Team
Advocate for the people supported and assist with developing community supports to promote optimal health for the people supported

Requirements

Graduation from an accredited licensed nurse program
Current nursing license with no restrictions to practice nursing.
Current CPR Certification or eligibility to obtain certification
Must be able to communicate effectively with Interdisciplinary Team Members, as well as the people supported and their families
Must be able to work independently, be organized, and possess the ability to prioritize work tasks

Physical requirements include: ability to regularly lift 20 pounds. Must be able to lift a minimum of 40 pounds. Must be able to pull a minimum of 25 pounds. Must be able to squat, kneel, crawl, crouch, climb, and stoop. Must be able to regularly stand and walk. Must be able to demonstrate proficiency in CPR from the floor level. Vision requirements include close vision, distance vision, and peripheral vision. Must be able to communicate.

Required Education/Experience Education Certified Registered Nurse/ LPN Experience Five years’ experience in the human services field working with individuals with behavioral and or mental health issues (preferred) Required Licenses/Certifications Valid driver’s license Responsibilities: Monitor healthcare needs of individuals receiving services and coordinate medical, dental , and vision care services and appointments. Provide assistance to individuals receiving services who are unable to self-administer medication and supervise self-administered medication and injection of medications, as prescribed. Educate individuals receiving services on the use of their medication including the purpose, side effects, and administration methods. Ensure compliance with all internal and external requirements and regulations regarding medication administration documentation and reporting. Prepare and complete all necessary documentation, including assessments, nurses notes, medication administration information, and miscellaneous paperwork/reports as requested .
