At Right at Home, we bring nursing care directly into our patients' homes. We are looking for a dedicated LPN (Home Health) to join our clinical team.
In this role, you will perform vital nursing care in accordance with an established plan of care, agency policies, and professional standards. Working under the supervision of a Registered Nurse (RN), you will act as a trusted clinical partner to our patients and their families.
How You Will Make an Impact (Essential Functions):
Deliver Hands-On Care: Perform assigned nursing procedures, including the administration of treatments and medications, under RN supervision and in accordance with the State Nurse Practice Act.
Be the Eyes and Ears: Observe patient conditions and family situations, immediately reporting any signs or symptoms indicative of changes to the Nursing Supervisor and documenting them in the clinical record.
Maintain Accurate Records: Prepare and record detailed clinical notes and progress notes.
Prioritize Safety: Prepare equipment and materials for treatments using strict aseptic techniques as required.
Empower Independence: Assist patients in learning appropriate self-care techniques and support their rehabilitation according to the patient care plan.
Collaborate Professionally: Consult the Nursing Supervisor when making judgments or decisions regarding patient care and treatments.
Stay Adaptable: Stay ready to perform additional tasks as requested by your supervisors to meet patient and agency needs.
What We Are Looking For (Requirements):
Education: Graduate of an approved Practical/Vocational Nursing Program.
Experience: Minimum of one (1) year of nursing experience within the past 5 years, OR completion of an accredited refresher course within the past 2 years.
Licensure: Currently licensed and in good standing as a Practical/Vocational Nurse in this State.
Clinical Excellence: Knowledge, skill, and judgment to perform all nursing procedures outlined in the Nurse Practice Act, alongside an understanding of the working team concept.
Transportation: Valid driver’s license and use of an insured automobile for work, or reliable access to adequate transportation.
Communication: Ability to read, write, speak, and understand English fluently as needed for the job.
Professional Growth: Active participation in state-mandated in-service education programs (can be met outside of Right at Home sponsored programs).
Working Conditions & Key Disclaimers:
Environment & Schedule: Primarily works in patients’ places of residence with travel required. Shift or visit times and days vary based on patient needs.
Hours & Locations: The Agency cannot promise to provide a certain number of visits or hours of employment in any given week. We cannot guarantee work close to your home or on a convenient public transportation line.
Quality Assurance: Right at Home utilizes remote camera observation and photography while you are at work as part of our quality control measures. Consent to being remotely observed and photographed is a requirement of the role.
