Nurse Coordinator describes a variety of specialized nursing positions that aid in planning, coordinating, and delivering patient care for selected patient populations across the continuum of care using the nursing process, ensuring compliance with physician orders and established policies and procedures. Nurse Coordinators provide coordination and care of patients requiring assistance and support in accessing and managing their health care needs. They collaborate with other health care providers to ensure continuity of care and coordination of a wide variety of patient services. They will use performance improvement methodology to design data collection systems / processes and identify opportunities to improve care/service for patients. Responsibilities include coordinating patient contact, patient education, and preparing patients / families for procedures/tests in collaboration with the health care team; collaborating with others to ensure continuity of care and coordination of a wide variety of patient services; identifying opportunities to improve patient care services; assisting management to identify and develop operational needs for the unit and to improve patient care services; and they may provide direct patient care.

  • Collaborates with inpatient and other staff to develop informational/educational materials appropriate for the patient population.
  • Develops, plans, and evaluates workshops, staff training/orientation and conferences.
  • Assesses learning needs of patient populations and develops curriculum accordingly.
  • Participates in formal and informal evaluation of staff competency.
  • Communicates with patients and their relatives to explain diagnostic and therapeutic measures related to condition, provide instruction, and promote participation in patient care.

Qualifications

MINIMUM QUALIFICATIONS

Any combination of education and experience that would likely provide the required knowledge, skills and abilities as well as possession of any required licenses or certifications is qualifying.

Education: Bachelor's Degree in Nursing from an accredited college or university.

Experience: Three (3) years of progressively responsible and directly related work experience.

License/Certification

  • Basic Life Support by American Heart Association (AHA)
  • or
  • RQI Healthcare Provider Basic Life Support
  • Registered Nurse by California Board of Registered Nursing

You may be entitled to paid travel time and reimbursement for mileage, pursuant to applicable law.
