What you'll do

The Nursing Assistant (NA) I is a key member to the nursing care team. They assist with direct and indirect patient care within their scope of practice, supporting patients, nursing and medical care teams. The care delivered is in collaboration and under the supervision/ delegation of the Registered Nurse (RN) and/or Licensed Practical Nurse (LPN)/ The Nursing Assistant (NA) shares accountability for the quality of patient outcomes. In select departments, the NA role may also include some clerical support functions (such as phone answering, filing, computer data entry, etc.)

  • Provides direct / non-direct patient care:
    • Performs Activity of Daily Living (ADL) care including, but not limited to:
  • Essential Daily Care:
    • Oral care, bathing, showering, shaving, hair washing and maintenance, incontinence care and toileting/elimination needs.
    • Changing bed linen.
    • Maintaining a decluttered patient care area.
    • Understanding available products for patient care and when to utilize.
  • Safety and Preventative Care:
    • Infection prevention.
    • Skin and pressure injury prevention.
    • Fall prevention.
  • Patient safety:
    • Setting and responding appropriately to patient safety alarms.
  • Environmental safety:
    • Understanding and performing regulatory compliance.
  • Patient Mobility:
    • Patient turning and offloading.
    • Ambulation, mobility in room, range of motion.
    • Understanding patient mobility assist devices and appropriate use of for patient safety.
    • Use of patient lift equipment as per Safe Patient Handling (SPH) policy.
  • Nutrition:
    • Assist with meal tray delivery, collection, and documentation of amount consumed.
    • Set up and assist patients with meals, including feeding.
  • Performs patient data collection, including but not limited to:
    • Use of data collection equipment: Vital signs, EKG, bladder scan, transfer devices, telemetry, Massimo
  • Documentation in EMR:
    • Components of vital signs, height and weight, intake and output measurement (including drains, urinary catheter and suction devices.)
    • Essential care performed.
    • Intentional rounding.
  • Provides direct patient monitoring, including but not limited to:
    • Safety Attendant (SA) at bedside for prevention of unintentional or intentional (suicide precautions) self-harm.
    • Video surveillance.
  • Performs patient care interventions as delegated and within scope of practice and validated competency, including but not limited to:
    • Phlebotomy, EKG’s, specimen collection, blood glucose measurement and telemetry.
  • Others patient care needs, including but not limited to:
    • Prepare patient rooms for admissions, discharges. Update whiteboards. Provide unit / room stocking of supplies.
    • Care of a patient with invasive lines and devices within NA scope of practice.
    • Transporting patients on/off unit.
    • Responding to emergency situation.
    • Communication with patient care team to ensure delivery of quality care.
    • Unit-specific / patient population specific interventions as delegated (example: psychiatry patients, pediatric patients, etc.)
  • Communicates effectively with nursing and healthcare team, including but not limited to:
    • Understands and reports out of range patient data values to covering RN.
    • Recognize and report patient changes to covering RN.
    • Performs shifts reports with oncoming staff.
    • Demonstrates initiative and understanding of job duties through communication with healthcare team and own self initiative.
    • Customer Service
    • Relating and respecting each patient individually.
  • Meeting needs of each patient to ensure excellent patient care is delivered. This includes a patient’s cognitive, physical and emotional needs, as well as their personal preference, cultural and religious beliefs.
  • Performs other duties as requested by healthcare and leadership team, within the scope of practice.

What we need from you

The primary function of the Adult Day Care (ADC) Aide is to provide your assigned registrants with routine daily care in accordance with our established care procedures as may be directed by your supervisor. Completing and performing your assigned tasks, must report any abnormal findings to the ADC Coordinator or designee. Perform all assigned tasks in accordance with the established policies and procedures and as instructed by your supervisor. Notify the facility when you will be late or absent from work as well as the ADC Coordinator. Report all complaints and grievances made by the registrant or registrant's family members. Cooperate with interdepartmental personnel, as well as other facility personnel to assure that services can be adequately maintained to meet the needs of the registrants. Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as common environment throughout your assigned shift. Participate in and receive a report upon reporting for duty. Perform those nursing functions as assigned by the ADC Coordinator. Report to duty on time and in proper attire. Practice proper infection control techniques. Complete assignments by end of shift. Attend to registrant's needs promptly Assist other Adult Day Care personnel as needed. Maintain a safe and clean environment for all registrants assigned to your care. Demonstrate the basic techniques of rehabilitation. (Ex; Range of motion, activities of daily living, ambulation and transfer techniques). Attend in-service programs as directed by the ADC Coordinator. Maintain records as directed. Check registrants routinely to ensure that their care needs are being met. Report any changes in room temperature, ventilation, lighting, etc. to the appropriate personnel. Assist registrants in preparing for meals and assist in serving meals to the registrants. Assist in serving between meal snacks. Pick up and clean environment after activities and meal time. Maintain a safe, attractive and clean work area. Operate all equipment in a safe manner. Maintain the confidentiality of all registrant care information. Ensure that you treat all registrants fairly, and with kindness, dignity and respect. Ensure that all care is provided in privacy as appropriate. Report all grievances and complaints made by the registrant to the ADC Coordinator. Must possess a valid NYS driver license without violations. Drive and/or monitor registrants on the facility van when transporting registrants to and from the Adult Day Health Care Program. Nurse Aide must attend to registrant's needs and safety. Nurse Aide must learn and use proper van hook-ups. Nurse Aide must supervise registrants on all outings. Nurse Aide must follow activity schedule and execute all plans on monthly calendar. Nurse Aide must initiate and continue activities. Nurse Aide will put all equipment and materials away at end of shift. If Adult Day Health Care closes for a snow day or another type of emergency day, the Adult Day Health Care Nurse Aides must report to the facility Nursing Supervisor for an assignment on the Nursing units. If Adult Day Health Care census is deemed low, the ADC Coordinator will assign Nurse Aide to the Nursing Home, Nurse Aide must report to the facility Nursing Supervisor for an assignment on the Nursing units. Attend all in-services and follow all department and facility policies including the use of personal protective equipment. Maintain CONFIDENTIALITY of all pertinent resident care and employee information and follow HIPAA and HIV regulations. Assists/ feeds residents with meals and supplements. Assists clients with elimination needs, and/or provides incontinent care with each occurrence. Transfers, moves and repositions clients every 2-3 hours and prn. Maintains CNA II Requirement (if appropriate). Assists residents to move to activities area or dining room area, etc. Keeps unit neat, relaxed and homelike. Assists with admissions, transfers and discharges. Assists/provides residents with PROM and AROM, and ambulation goals as appropriate. Provides treatments as ordered on assigned residents. Operates all equipment and performs all procedures/care in a safe manner. Demonstrates proper body mechanics in all functions. Cleans and stocks supplies, and equipment as directed. Receives and directs phone calls promptly and accurately following Hospital etiquette guidelines. Demonstrates ability to accept constructive criticism and uses it to improve work performance. Demonstrates the skills and judgment necessary to provide non–direct patient care/support services to staff under the direction of licensed personnel. Maintains NYS CNA requirements. Promotes effective working relations and works effectively as part of a department/unit team inter and intra departmentally to facilitate the department's/unit's ability to meet its goals and objectives. Assumes all other duties and responsibilities as necessary. Completes work assignments on time/readily accepts assignments as observed by Director.
