We are in need of a new Public Health Nurse Sr. (PHN Sr.), based in our Fluvanna County Health Department. Our ideal candidate would be a practicing nurse with public health or community outreach experience, and program leadership capability. This position is highly mobile, where you will work throughout all district clinics providing immunization, tuberculosis screening and management, communicable disease investigation, Newcomer Health assessments, and contraception and sexual health services for our community members. This PHN Sr. role also conducts in home screenings for adults and children to determine eligibility for Medicaid long term services, in addition to providing clinic-based childhood school and sports physicals. Minimum Qualifications

• Must be certified in Basic Life Support (BLS)/CPR, able to be fit tested and perform fit testing for other staff in accordance with OSHA Respiratory Protection Standard (29 CFR 1910.134) [Ref.2] regulations

• Requires licensure as an RN in the state of Virginia.

• Experience functioning in a leadership role; experience and ability to function independently in the home/clinic/community/school settings and prioritize workload

• Exercises sound nursing judgment; effective verbal, written, interpersonal/physical assessment skills

• Experience and ability to enter, review, analyze data and statistics and produce reports and/or recommendations from such information; ability to work in a fast paced, data driven environment

• Experience and knowledge of nursing process, health maintenance, disease processes, physical and behavioral sciences, case management process and basic principles of epidemiology

• Work effectively as a member of the multidisciplinary health care team

• Knowledge and experience in use of computers and computer applications; including Internet, Microsoft Office, and Google Mail applications

• Experience and ability to work with diverse cultural backgrounds

• May travel and some weekend and/or evening hours may be required

• Excellent communication, data interpretation and computer skills

• Must be able to demonstrate decision making skills, teaching, interviewing, counseling skills and the ability to analyze problems/situations and problem solve

The Blue Ridge Health District (BRHD) serves the people in the following counties: Albemarle, Fluvanna, Greene, Louisa, Nelson, and the City of Charlottesville. We are in need of a new Public Health Nurse (PHN) based in our Charlottesville/Albemarle Health Department. Our ideal candidate would be a practicing nurse with public or community health experience. This position is highly mobile, where you will work throughout all district clinics providing immunization, tuberculosis screening and management, communicable disease investigation, Newcomer Health assessments, contraception and sexual health services to our community. This PHN role also conducts in home screenings for adults and children to determine eligibility for Medicaid long term services, in addition to providing clinic-based childhood school and sports physicals. VDH requires all Health-Care Personnel (HCP) to provide documentation of immunization or evidence of immunity to the following: hepatitis B, measles, mumps, rubella, and varicella; a one-time dose of tetanus, diphtheria, and pertussis (Tdap) vaccine regardless of the interval since the last dose of tetanus-diphtheria (Td) vaccine was administered (Td or Tdap boosters shall be received every 10 years); and the current year's seasonal influenza vaccination. Wage employees are subject to the following conditions of employment: • Wage employees serve at the will and pleasure of the appointing authority. • Continuing employment is based on available funding and business need. • Wage employees are not covered by the provisions of the Virginia Personnel Act. Therefore, they are not eligible for benefits which are provided to salaried employees, such as leave credits, holiday pay, insurance, access to the grievance procedure, etc. • Wage employees are paid only for the hours they actually work. • Virginia state law currently requires that agencies not permit wage employees to work in excess of 29 hours per week on average over the course of a year. Minimum Qualifications • Requires licensure as an RN in the state of Virginia. • Certified in BLS/CPR. • Experience and ability to function independently in the home/clinic/community/school settings. • Exercises sound nursing judgment. • Experience and ability to enter, review, and analyze data. • Experience and knowledge of nursing process, health maintenance, disease processes, and case management. • Work effectively as a member of the multidisciplinary health care team. • Knowledge and experience in use of computers and computer applications (including Microsoft Office and Google Mail applications). • Experience and ability to work with diverse cultural backgrounds. • May travel and some weekend and/or evening hours may be required. • Must be able to demonstrate decision-making skills, teaching, interviewing, counseling skills and the ability to analyze problems/situations and problem solve.
